摘要
目的观察自制乐方天麻钩藤乐曲结合针刺治疗肝阳上亢型偏头痛急性发作期的临床效果。方法将2019年12月—2021年3月就诊于中国中医科学院广安门医院的180例肝阳上亢型偏头痛急性发作期患者按随机数字表法分为3组,针刺组55例、音乐组56例、针刺联合音乐组55例完成研究。针刺组采用针刺常规穴位治疗,音乐组采用静听自制天麻钩藤乐曲治疗,针刺联合音乐组采用针刺常规穴位结合自制天麻钩藤乐曲治疗。观察3组患者治疗后1 h、2 h和24 h的头痛VAS评分,检测3组治疗前及治疗后24 h血清5-羟色胺(5-HT)和降钙素基因相关肽(CGRP)水平,比较3组治疗效果及不良反应发生情况。结果治疗后3组VAS评分时间点比较及交互作用差异均有统计学意义(P均<0.05)。3组治疗后1 h、2 h、24 h的VAS评分均明显低于治疗前(P均<0.05),针刺联合音乐组治疗后24 h的VAS评分明显低于同期针刺组和音乐组(P均<0.05)。治疗后24 h,3组血清5-HT水平均明显升高(P均<0.05),血清CGRP水平均明显降低(P均<0.05),但3组间治疗后24 h血清5-HT和CGRP水平比较差异均无统计学意义(P均>0.05)。针刺联合音乐组总有效率为92.7%(51/55),明显高于针刺组[81.8%(45/56)]和音乐组[73.2%(41/55)],差异均有统计学意义(P均<0.05),针刺组和音乐组比较差异无统计学意义(P>0.05)。3组均未出现明显不良反应。结论自制中医五音乐方天麻钩藤乐曲结合针刺治疗肝阳上亢型偏头痛急性发作期的临床效果优于单纯针刺和单纯天麻钩藤乐曲治疗。
Objective It is to observe the clinical effect of self-made Tianma Gouteng music combined with acupuncture on acute attack of migraine of type of liver-Yang hyperactivity.Methods A total of 180 patients with acute attack of migraine of type of liver-Yang hyperactivity treated in Guang’an Men Hospital,China Academy of Chinese Medical Science from December 2019 to March 2021 were randomly divided into 3 groups according to the random number table method,55 cases in the acupuncture group,56 cases in the music group and 55 cases in the acupuncture plus music group completed the study.The acupuncture group was treated with acupuncture at normal used acupoints,the music group was treated with quietly listening of self-made Tianma Gouteng music,the acupuncture plus music group was treated with both acupuncture at normal used acupoints and Tianma Gouteng music.The headache scores of visual analogue scale(VAS)of the patients in the 3 groups at 1 h,2 h and 24 h after treatment were observed,the levels of serum 5-hydroxytryptamine(5-HT)and calcitonin gene related peptide(CGRP)before and 24 h after treatment were determined,the treatment effects and the occurrences of side effect were compared among the 3 groups.Results After treatment,the differences in the VAS scores at time point and the difference of interaction among the three groups were statistically significant(all P<0.05).The VAS scores at 1 h,2 h and 24 h after treatment in the 3 groups were significantly lower than those before treatment(all P<0.05),and the VAS scores at 24 hours after treatment in the acupuncture plus music group were significantly lower than those in the acupuncture group and music group at the same time(all P<0.05).24 hours after treatment,the serum 5-HT levels in the 3 groups were significantly increased(all P<0.05)and the serum CGRP levels were significantly decreased(all P<0.05),but there was no significant difference in the serum 5-HT and CGRP levels among the 3 groups 24 hours after treatment(all P>0.05).The total effective rate of the acupuncture plus music group was 92.7%(51/55),which was significantly higher than that 81.8%(45/56)of the acupuncture group and 73.2%(41/55)of the music group,and the differences were statistically significant(both P<0.05),there was no significant difference between the acupuncture group and the music group(P>0.05).No obvious adverse reaction occurred in the 3 groups.Conclusion The efficacy of self-made Tianma Gouteng music combined with acupuncture is better than simple acupuncture and simple self-made Tianma Gouteng music in the treatment of acute attack of migraine of type of liver-Yang hyperactivity.
