摘要
目的:探讨胰岛素四针强化注射与预混胰岛素治疗2型糖尿病的临床效果。方法:选取2018年1月-2021年10月增城区小楼镇卫生院收治的2型糖尿病患者80例作为研究对象,根据随机抽样法分为对照组与观察组,各40例。对照组采用两针预混胰岛素治疗,观察组采用四针强化注射(一针长效联合三针短效)胰岛素治疗。比较两组临床疗效、临床指标、低血糖发生率。结果:两组患者总有效率比较,差异无统计学意义(P>0.05)。治疗后,两组患者空腹C肽、餐后1 h C肽水平均升高,且观察组高于对照组,差异有统计学意义(P<0.05);治疗后,两组患者糖化血红蛋白、空腹血糖、餐后2 h血糖水平均降低,且观察组低于对照组,观察组血糖达标时间短于对照组,差异有统计学意义(P<0.05)。两组患者低血糖发生率比较,差异无统计学意义(P>0.05)。结论:胰岛素四针强化注射或两针预混胰岛素注射均可提高2型糖尿病的整体疗效,在控制机体血糖水平的同时稳定C肽水平,降低低血糖发生率,但胰岛素四针强化注射的整体效果更理想。
Objective:To compare the clinical effect of intensive insulin four injections and premixed insulin in treatment of type 2 diabetes mellitus.Methods:From January 2018 to October 2021,80 patients with type 2 diabetes mellitus admitted to Zengcheng District Xiaolou Town Health Center were selected as the study subjects,and they were divided into control group and observation group according to random sampling method,with 40 cases in each group.The control group was treated with two injections of premixed insulin,and the observation group was treated with intensive four injections of insulin(one long-acting injection combined with three short-acting injections).The clinical efficacy,clinical indicators and incidence of hypoglycemia were compared between the two groups.Results:There was no significant difference in the total effective rate between the two groups(P>0.05).After treatment,the fasting C-peptide and 1 h postprandial C-peptide levels in both groups increased,the observation group was higher than the control group,with a statistically significant difference(P<0.05).After treatment,the glycosylated hemoglobin,fasting blood glucose,and 2 h postprandial blood glucose levels in both groups decreased,the observation group was lower than the control group,and the blood glucose compliance time in the observation group was shorter than that in the control group,with a statistically significant difference(P<0.05).There was no significant difference in the incidence of hypoglycemia between the two groups(P>0.05).Conclusion:The choice of intensive insulin four injections and two injections of premixed insulin can improve the overall efficacy of type 2 diabetes mellitus,stabilize C-peptide level while controlling blood glucose levels,and reduce the incidence of hypoglycemia,but the overall effect of intensive insulin four injections is more ideal.
