摘要
目的分析西安大兴医院临床输血不良事件及输血反应报告,提出预防改进措施,实现血液安全预警。方法按照发生原因、发生时间段、事件严重程度对西安大兴医院各部门2020年6月—2022年6月通过办公自动化(office automation,OA)网络信息平台上报的37例临床输血不良事件进行回顾性分析,按照发生类型、输血血液成分种类对本院2020年6月—2022年6月通过输血管理信息系统上报的51例输血反应进行回顾性分析。结果上报的临床输血不良事件分级主要是Ⅲ级,共25例,占67.57%;临床输血不良事件前3位发生原因共占67.57%;发生时间段主要集中在夜间值班和上午下班前1 h,共占81.08%。上报的输血反应共51例,均为非感染性反应,主要发生于红细胞输注。其中非溶血性发热反应27例,占52.94%,过敏反应23例,占45.10%。结论临床输血不良事件及输血反应的风险管理、积极上报与分析有助于降低其发生率,保障患者用血安全。
Objective To analyze the reports of adverse blood transfusion events and transfusion reactions in Xi'an Daxing Hospital,put forward the preventive and improvement measures to realize the early warning of blood safety.Methods A retrospective analysis was conducted on 37 clinical adverse events of blood transfusion reported by various departments of Xi'an Daxing Hospital from June 2020 to June 2022 through the office automation(OA)network information platform according to the causes,time periods and severity of the events.A retrospective analysis of 51 cases of transfusion reactions reported through the transfusion management information system from June 2020 to June 2022 was conducted according to the types of occurrence and blood components of transfusion.Results The grade of adverse blood transfusion events was mainlyⅢ,25 cases(67.57%).The first 3 causes of adverse blood transfusion events accounted for 67.57%.The occurrence time was mainly on duty at night and one hour before the end of work in the morning,accounting for 81.08%.A total of 51 cases of blood transfusion reactions were reported,all of which were non-infectious reactions and mainly occurred from red blood cell transfusion.Among them,27 cases were non-hemolytic fever reaction(52.94%)and 23 cases were allergic reaction(45.10%).Conclusion The risk management,active reporting and analysis of clinical transfusion adverse events and transfusion reactions are helpful to reduce the incidence and ensure the blood safety of patients.
