摘要
目的 探讨DR患者血红蛋白糖化指数(HGI)和UACR变化的意义。方法 选取2019年1月至2020年12月于宿迁市第一人民医院内分泌科住院的T2DM患者268例。根据眼底检查结果分为单纯T2DM组(T2DM,n=159)及T2DM合并DR组(DR,n=109)。根据FPG和HbA1c计算HGI,定义HGI≥0为高水平,HGI<0为低水平。比较两组一般资料和实验室生化指标,分析DR的危险因素;比较两组高HGI、UACR发生率及HGI+UACR对DR的诊断价值。结果 Logistic回归分析结果显示,HGI、UACR是DR的危险因素。DR组高HGI、高UACR发生率高于T2DM组(P<0.05)。受试者工作特征(ROC)曲线分析显示,HGI+UACR可有效预测DR,ROC曲线下面积为0.76(P<0.01)。结论 HGI与UACR与DR发病密切相关,对DR有一定预测价值,HGI联合UACR可用于DR个性化评估和风险预测。
Objective To investigate the changes and clinical value of hemoglobin glycation index(HGI)and urine albumin/creatinine ratio(UACR)in in patients with type 2 diabetic retinopathy(DR).Methods A total of 268 patients with type 2 diabetes mellitus(T2DM)who were hospitalized in the Endocrinology Department of our hospital were enrolled in this study from January 2019 to December 2020.According to the results of fundus examination,the patients were divided into two groups:simple T2DM group(n=159)and DR(n=109). HGI was calculated based on FPG and Hb A1c. High HGI was defined as HGI ≥ 0,and low HGI was defined as HGI<0. The general data and laboratory indicators were compared between the groups,and the risk factors for DR were analyzed. The incidence of high HGI and high UACR were compared between the two groups. The diagnostic value of HGI+UACR for DR was evaluated.Results Logistic regression analysis showed that HGI and UACR were risk factors for DR.The incidence of high HGI and high UACR was higher in DR group than in T2DM group(P<0. 05). The results of ROC curve analysis showed that HGI+UACR could effectively predict DR,and the area under the curve was 0. 76(P<0. 01).Conclusion HGI and UACR are closely related to the onset of DR in T2DM patients,and have certain value in the prediction of DR. HGI combined with UACR is expected to be used for personalized disease assessment and risk prediction for DR.
