摘要
目的 探讨低分子肝素钙注射液(商品名:速碧林)联合低分子右旋糖酐对胎儿宫内生长发育的影响。方法 50例胎儿生长受限的孕妇,随机分为对照组和治疗组,每组25例。对照组采用常规治疗方法,治疗组在对照组基础上加用速碧林联合低分子右旋酐糖治疗。比较两组治疗前后胎儿动力指数(PI)、阻力指数(RI)、收缩期血液流速高值/舒张晚期血液流速高值(S/D值)和双顶径(BPD),新生儿出生体重、胎盘重量、头围(HC)、腹围(AC)、股骨长(FL)。结果 治疗后,两组PI、RI、S/D值均较治疗前降低,BPD均较治疗前增加,且治疗组PI(0.79±0.08)、RI(0.54±0.03)、S/D值(2.67±0.29)低于对照组的(0.93±0.28)、(0.61±0.06)、(2.95±0.41),BPD(8.98±0.14)cm大于对照组的(8.14±0.37)cm,差异有统计学意义(P<0.05)。治疗组新生儿出生体重(2800.34±220.64)g、胎盘重量(500.36±20.49)g、HC(28.51±0.42)cm、AC(27.46±0.63)cm、FL(5.92±0.35)cm均大于对照组的(2590.46±264.14)g、(400.36±30.27)g、(27.94±0.45)cm、(26.82±0.54)cm、(5.63±0.32)cm,差异有统计学意义(P<0.05)。结论 速碧林联合低分子右旋糖酐治疗胎儿宫内生长受限,能改善胎盘血流灌注,明显改善脐血流S/D值,恢复正常胎盘功能,增加对母体营养物质的吸收以及对能量的摄入,更好地增加胎儿体重,促进胎儿发育。
Objective To discuss the effect of low molecular weight heparin calcium injection(trade name:Subilin) combined with low molecular dextran on intrauterine fetal growth and development.Methods A total of 50 pregnant women with fetal growth restriction were randomly divided into the control group and the treatment group,with 25 cases in each group.The control group was treated with conventional methods,while the treatment group was treated with Subilin and low molecular dextran sugar on the basis of the control group.Both groups were compared in terms of pulsatility index(PI),resistance index(RI),peak systolic velocity/end diastolic velocity(S/D) and biparietal diameter(BPD),neonatal birth weight,placental weight,head circumference(HC),abdominal circumference(AC),and femoral length(FL).Results After treatment,the PI,RI,and S/D decreased and BPD increased in both groups compared to those before treatment in this group;the PI(0.79±0.08),RI(0.54±0.03),and S/D(2.67±0.29) in the treatment group were lower than(0.93±0.28),(0.61±0.06),and(2.95±0.41) in the control group;the BPD(8.98±0.14) cm in the treatment group was greater than(8.14±0.37) cm in the control group;the differences were all statistically significant(P<0.05).The birth weight(2800.34±220.64) g,placental weight(500.36±20.49) g,HC(28.51±0.42) cm,AC(27.46±0.63) cm,and FL(5.92±0.35) cm in the treatment group were greater than(2590.46±264.14) g,(400.36±30.27) g,(27.94±0.45) cm,(26.82±0.54) cm,and(5.63±0.32) cm in the control group,and the differences were all statistically significant(P<0.05).Conclusion Subilin combined with low molecular dextran in the treatment of intrauterine growth restriction can improve placental blood perfusion,significantly improve umbilical blood flow S/D value,restore normal placental function,increase maternal absorption of nutrients,increase energy intake,better increase fetal body weight and promote fetal development.
