摘要
目的 研究女性HPV(人乳头瘤病毒)感染与宫颈癌前病变相关性。方法 采取前瞻性研究,以进行宫颈活检的112例患者作为研究对象,通过液基细胞学(1iquid based thin prepcytologytes, LCT)检测患者宫颈组织细胞样本的病变情况,并对患者的HPV感染进行检测。分析不同HPV感染患者的宫颈病变情况、不同宫颈病变患者的HPV感染亚型以及HBV病毒载量情况之间的差异,研究宫颈CINⅠ~Ⅲ病变与HPV感染以及HPV16感染的相关性。结果 通过对不同HPV感染患者的宫颈病变分析,宫颈CINⅠ~Ⅲ病变患者的HPV感染率显著高于无细胞学改变、炎性细胞反应患者(χ^(2)=46.821,P=0.000);宫颈CINⅠ~Ⅲ病变患者主要以HPV16感染为主(χ^(2)=46.821,P=0.000)。无细胞学改变与宫颈CINⅠ~Ⅲ病变患者的HPV16的病毒载量之间的差异不存在统计学意义(P>0.05)。宫颈CINⅠ~Ⅲ病变与HPV感染及HPV16感染呈正相关(P<0.05)。结论 女性HPV感染与宫颈癌前病变显著相关,在对患者的细胞学形态检查的基础上联合HPV检查,对于宫颈癌前病变诊断具有重要意义。
Objective To study the relationship between HPV(human papillomavirus)infection and cervical precancerous lesions.Methods This study was a prospective study.112 patients who had undergone cervical biopsy were taken as the study objects.The pathological changes of cervical tissue and cell samples of patients were detected by liquid based cytology(LCT),and the HPV infection of patients was detected.To analyze the differences between cervical lesions of different HPV infected patients, HPV infection subtypes and HBV viral load of different cervical lesions, and to study the correlation between cervical CINⅠ~Ⅲlesions, HPV infection and HPV16 infection.Results Through the analysis of cervical lesions in patients with different HPV infections, the HPV infection rate in patients with CINⅠ~Ⅲlesions was significantly higher than that in patients without cytological changes and inflammatory cell reaction(χ^(2)=46.821,P=0.000).Patients with CINⅠ~Ⅲlesions of the cervix are mainly infected with HPV16(χ^(2)=46.821,P=0.000).There was no significant difference between HPV16 viral load of patients without cytological changes and CINⅠ~Ⅲlesions(P>0.05).Cervical CINⅠ~Ⅲlesions were positively correlated with HPV infection and HPV16 infection(P<0.05).Conclusion HPV infection in women is significantly related to cervical precancerous lesions.On the basis of cytological examination of patients, combined with HPV examination, is of great significance for cervical precancerous lesions.
