摘要
目的比较经剑突下切口电视胸腔镜手术(S-VATS)和经侧胸切口电视胸腔镜手术(I-VATS)治疗前纵隔病变的近期效果。方法选择我院2015年12月至2020年12月收治的88例前纵隔病变患者为研究对象,按照随机数字表法将其分为A组(n=44,采取I-VATS治疗)和B组(n=44,采取S-VATS治疗)。比较两组的临床病情控制情况、炎症因子水平、免疫功能指标。结果术后7 d,B组的临床病情控制有效率高于A组,差异具有统计学意义(P<0.05)。术后3 d,B组的白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)水平低于A组,差异具有统计学意义(P<0.05)。术后7 d,B组的白细胞分化抗原3阳性(CD3^(+))、白细胞分化抗原4阳性(CD4^(+))高于A组,白细胞分化抗原8阳性(CD8^(+))低于A组,差异具有统计学意义(P<0.05)。结论采用S-VATS治疗前纵隔病变可取得更佳的治疗效果,有利于减轻炎症反应、改善免疫功能。
Objective To compare the short-term effects of subxiphoid approach video-assisted thoracoscopic surgery(S-VATS)and intercostal approach video-assisted thoracoscopic surgery(I-VATS)in the treatment of anterior mediastinal lesions.Methods Eighty-eight patients with anterior mediastinal lesions admitted in our hospital from December 2015 to December 2020 were selected as the research objects and divided into group A(n=44,treated with I-VATS)and group B(n=44,treated with S-VATS)according to random number table method.The clinical condition control,levels of inflammatory factors and immune function indexes were compared between the two groups.Results At 7 d after operation,the effective rate of clinical condition control in the group B was higher than that in the group A,and the difference was statistically significant(P<0.05).At 3 d after operation,the levels of interleukin-1β(IL-1β),interleukin-6(IL-6),C-reactive protein(CRP)and tumor necrosis factor-α(TNF-α)in the group B were lower than those in the group A,and the differences were statistically significant(P<0.05).At 7 d after operation,cluster of differentiation 3 positive(CD3^(+))and cluster of differentiation 4 positive(CD4^(+))in the group B were higher than those in the group A,cluster of differentiation 8positive(CD8^(+))was lower than that in the group A,and the differences were statistically significant(P<0.05).Conclusion S-VATS in the treatment of anterior mediastinal lesions can achieve better therapeutic effect,it is conducive to reducing inflammatory response,improving immune function.
