摘要
目的:评价改良劈核法小切口非超声乳化术治疗硬核白内障患者的临床疗效。方法:选取芜湖市眼科医院2018年10月~2021年7月收治的36例老年性硬核白内障患者,随机分为对照组和观察组,每组18例。对照组行超声乳化吸除术,观察组采用改良劈核法小切口非超声乳化摘除术。比较两组患者术后1天和3个月的视力、散光、角膜内皮计数、并发症等情况。结果:两组患者术后1天、3个月视力均较治疗前改善(P<0.05),且观察组患者视力恢复优于对照组(P<0.05)。两组患者术后3个月散光均较治疗前改善(P<0.05),但两组患者术后散光改善程度差异无统计学意义(P>0.05)。两组患者手术后角膜内皮数均较治疗前减少(P<0.05),且观察组患者角膜内皮数变化幅度小于对照组(P<0.05)。观察组并发症总发生率(22.22%)低于对照组(61.11%)(P<0.05)。结论:应用改良劈核法小切口非超声乳化术治疗高龄硬核白内障患者,术后视力提高明显,且术后角膜内皮损伤小,无严重并发症的发生,效果显著,值得推广。
Objective:To evaluate the clinical effect of modified nuclear fragmentation technique and small incision non-phacoemulsification cataract surgery for the treatment of hard nuclear cataract.Methods:Thirty-six elderly patients with hard nuclear cataract treated in our hospital were included from October 2018 to July 2021,and randomized to control group and observational group(n=18 for each group).Patients in the control group received phacoemulsification,and those in the observational group were treated with modified small incision non-phacoemulsification.The two groups were compared regarding the visual acuity,astigmatism,corneal endothelial count and complications at day 1 day and month 3 after operation.Results:The visual acuity of the two groups was significantly improved at day 1 day and month 3 following surgery(P<0.05),and the visual acuity recovery was better in the observational group than in the control group(P<0.05).Patient′s astigmatism in the two groups was significantly improved 3 months after operation(P<0.05),yet the improvement was not significantly different between groups(P>0.05).The number of corneal endothelial cells was decreased in the two groups after surgery compared to that before treatment(P<0.05),and the changes in the number of corneal endothelial cells were significantly less in the observational group than in the control group(P<0.05).Patients in the observational group had fewer total incidence of complications than those in control group(22.22%vs.61.11%,P<0.05).Conclusion:The modified nuclear fragmentation and small incision non-phacoemulsification is worthy of clinical recommendation in the treatment of hard nuclear cataract in elderly patients,because this surgery can significantly improve the postoperative visual acuity,with minimal injury to the corneal endothelia and without no serious complications.
