摘要
目的:观察补肾助孕方治疗肾虚型多囊卵巢综合征(PCOS)的疗效及其对性激素、转化生长因子-β1(TGF-β1)、胰岛素样生长因子-1(IGF-1)和胰岛素生长因子结合蛋白-3(IGFBP-3)的影响。方法:选择2018年7月—2021年6月在本院门诊就诊符合PCOS的患者72例,根据随机数字法将患者分为观察组和对照组,每组36例。对照组予以西药治疗,观察组在对照组的基础上使用补肾助孕方。观察两组治疗6个月经周期后的疗效,统计受孕率,治疗前后观察卵巢体积、卵泡直径、子宫内膜厚度、促黄体生成素(LH)、卵泡刺激素(FSH)、LH/FSH、睾酮(T)、空腹血糖(FBG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、胰岛素敏感指数(ISI)、TGF-β1、IGF-1和IGFBP-3水平的变化。结果:观察组的总有效率为91.7%,对照组的总有效率为69.4%,观察组的总有效率明显高于对照组(χ^(2)=4.596,P<0.05),观察组的妊娠率为41.7%,明显高于对照组的16.67%(χ^(2)=3.923,P<0.05)。两组治疗前卵巢体积、卵泡直径、子宫内膜厚度、LH、FSH、LH/FSH、T、FPG、FINS、HOMA-IR、ISI、TGF-β1、IGF-1和IGFBP-3水平差异均无统计学意义(P>0.05),治疗后两组卵巢体积、LH、LH/FSH、T、FPG、FINS和HOMA-IR、TGF-β1、IGF-1和IGFBP-3水平较治疗前明显降低(P<0.01),卵泡直径、子宫内膜厚度和ISI水平较治疗前升高(P<0.01),观察组与对照组比较降低或者升高水平均更为明显(P<0.01),两组患者的FSH水平治疗前后无明显变化(P>0.05)。结论:补肾助孕方治疗肾虚型PCOS患者疗效显著,能够调节性激素紊乱,降低血清TGF-β1、IGF-1和IGFBP-3水平。
Objective:To observe the clinical effect of Bushen Zhuyun Decoction in the treatment of polycystic ovary syndrome(PCOS)with kidney deficiency type,and its impacts on the levels of sex hormones,transforming growth factor-β1(TGF-β1),insulin-like growth factor-1(IGF-1)and insulin growth factor binding protein-3(IGFBP-3).Methods:72 patients with PCOS in the outpatient clinic of our hospital from July 2018 to June 2021 were divided into observation group and control group,with 36 cases in each group according to the random number method.The control group was treated with Western medicine,and the observation group was treated with Bushen Zhuyunfang Decoction on the basis of control group.The curative effect was observed,pregnancy rate was recorded,the ovarian volume,number of antral follicles,endometrial thickness,luteinizing hormone(LH),and follicle stimulating hormone(FSH),LH/FSH,testosterone(T),fasting blood glucose(FBG),fasting insulin(FINS),insulin resistance index(HOMA-IR),insulin sensitivity index(ISI),TGF-β1,IGF-1 and IGFBP-3 levels were compared between two groups before and after treatment.Results:The total effective rate was 91.7% in the observation group,which was significantly better than 69.4% in the control group(χ^(2)=4.596,P<0.05),the pregnancy rate in the observation group(41.7%)was significantly higher than that in the control group(16.7%)(χ^(2)=3.923,P<0.05).There was no significant difference in ovarian volume,follicle diameter,endometrial thickness,FSH,LH,LH/FSH,T,FPG,FINS,HOMA-IR,ISI,TGF-β1,IGF-1 and IGFBP-3 levels before treatment between the two groups(P>0.05),after treatment,the ovarian volume,number of antral follicles,LH,LH/FSH,T,FPG,FINS and HOMA-IR,TGF-β1,IGF-1 and IGFBP-3 levels were significantly lower in the two groups to compare with before treatment(P<0.01),the follicle diameter,endometrial thickness and ISI levels in the two groups were significantly higher to compare with before treatment(P<0.01);compared with the control group,the decreased or increased levels of above indexes were more obvious in the observation group(P<0.01),there was no significant changes on FSH levels between the two groups before and after treatment(P>0.05).Conclusion:The curative effect of Bushen Zhuyun Decoction in the treatment of PCOS with kidney deficiency type is obvious,which can significantly regulate sex hormone disorders,and reduce serum TGF-β1,IGF-1 and IGFBP-3 levels.
