摘要
目的 探究初产妇出院准备度在健康行为能力及其育儿胜任感中的中介效应,旨在为医院开展干预服务提供理论基础。方法 选取2022年3月至2022年5月产科初产妇650例,采用出院准备度调查表、健康行为能力量表、育儿胜任感量表调查初产妇出院准备度、健康行为能力、育儿胜任感。比较不同人口学特征初产妇育儿胜任感评分,同时采用皮尔逊相关性分析出院准备度、健康行为能力、育儿胜任感三者之间的关系,运用依次检验法检验出院准备度在健康行为能力及其育儿胜任感中的中介效应,并用Bootstrap法进行验证。结果 642例初产妇育儿胜任感评分为(79.89±7.10)分;不同学历、是否参加健康宣教的初产妇育儿胜任感评分比较,差异显著(P<0.05);出院准备度与健康行为能力、育儿胜任感评分呈显著正相关(r1=0.799,r2=0.834,均P<0.001),健康行为能力与育儿胜任感呈正相关关系(r=0.804,P<0.001);健康行为能力对初产妇育儿胜任感的直接效应显著,效应值为0.410,效果量为64.47%;出院准备度在健康行为能力与育儿胜任感之间的中介作用显著,效应值为0.226,效果量为35.53%。结论 初产妇育儿胜任感有待提高,学历、是否参加健康宣教影响其育儿胜任感,健康行为能力对初产妇育儿胜任感有直接影响,出院准备度可通过调节健康行为能力间接影响初产妇育儿胜任感,医护人员可制定针对性防护措施,以进一步改善预后。
Objective To explore the mediating effect of primipara discharge readiness on health behavior ability and parenting competence, in order to provide a theoretical basis for hospitals to carry out intervention services. Methods A total of 650 primiparas from March 2022 to May 2022 were selected, and the discharge readiness questionnaire, healthy behavior ability scale, and parenting competency scale were used to investigate the discharge readiness, healthy behavior ability, and parenting competency of primiparas. The parenting competency scores of primiparas with different demographic characteristics were compared, and Pearson correlation was used to analyze the relationship between discharge readiness, healthy behavior ability, and parenting competency. The mediating effect of parenting competence was verified by Bootstrap method. Results The parenting competency score of 642 primiparas was(79.89±7.10) points. The parenting competency scores of primiparas with different educational backgrounds and whether they participated in health education were significantly different(P<0.05).Competency score was significantly positively correlated(r1=0.799, r2=0.834, all P<0.001), and healthy behavioral ability was positively correlated with parenting competence(r=0.804, P<0.001). The direct effect of healthy behavior ability on primipara’s parenting competence was significant, the effect value was 0.410, and the effect size was 64.47%. The mediating effect of readiness for hospital discharge between healthy behavior ability and parenting competence was significant, with an effect value of 0.226 and an effect size of 35.53%. Conclusion Primitive mothers’ parenting competence needs to be improved.Educational background and whether they participate in health education and education affect their parenting competence.Health behavior ability has a direct impact on primipara’s parenting competence. Discharge readiness can indirectly affect primipara’s parenting competence by adjusting health behavior ability, medical staff can develop targeted protective measures to further improve the prognosis.
