摘要
目的通过系统评价再评价了解高强度间歇训练对心脏康复患者峰值摄氧量(peak oxygen uptake,VO2peak)、生活质量及运动安全的影响,为临床工作者制定锻炼方案提供循证依据。方法检索Web of Science、PubMed、the Cochrane Library、中国知网、万方和维普数据库,搜集建库至2021年4月19日关于高强度间歇训练(high-intensity interval training,HIIT)对心脏康复患者VO2peak、生活质量及运动安全影响的文献,采用AMSTAR 2工具评价纳入研究的方法学质量,采用GRADE系统评价证据质量。结果共纳入23篇文献,合计纳入研究项目及患者数分别为319项和14082例。AMSTAR 2量表评价显示,17.4%的文献为高质量,78.3%为中等质量,4.3%为低质量。GRADE系统分级证据质量显示,6.9%的结局指标证据质量为高,17.3%证据质量为中,37.9%证据质量为低,37.9%证据质量为极低。结论HIIT可明显提高心脏康复患者VO2peak;HIIT对生活质量的短期影响与疾病和训练方案有关,长期影响仍需进一步验证;HIIT用于心脏康复是安全和可行的。
Objective To perform an overview of the effects of high-intensity interval training(HIIT)on peak oxygen uptake(VO2peak),quality of life,and safety of the patients with cardiac rehabilitation so as to provide a reference for clinical staff to develop programs for cardiac rehabilitation exercise.Methods For this meta-analyses,databases of Web of Science,PubMed,the Cochrane Library,CNKI,WanFang Data,and VIP were searched,from inception through 19 April 2021,for collecting systematic reviews in the effects of high-intensity interval training on peak oxygen uptake,quality of life and safety of cardiac rehabilitation patients.The AMSTAR 2 was used to evaluate the methodological quality and GRADE to evaluate the quality of evidence.Results A total of 23 papers involving 319 studies and 14,082 patients were included.The results of the AMSTAR 2 evaluation showed that 17.4%of the systematic reviews were in high quality,78.3%were in moderate quality,and only 4.3%were in low quality.The results of the GRADE scores in the quality of evidence showed that 6.9%of the outcomes were in high quality of evidence,17.3%in moderate quality of evidence,37.9%in low quality of evidence,and 37.9%in very low quality of evidence.Conclusions HIIT significantly improves VO2peak in the patients undergoing cardiac rehabilitation.The short-term effects of HIIT on quality of life are related to disease and training programs.But the long-term effects requires further studies.HIIT is safe and feasible for cardiac rehabilitation.
