超声在卵巢病变良恶性病变鉴别诊断中的应用价值研究 被引量:1

The application value of ultrasound in the differential diagnosis of benign and malignant ovarian lesions

摘要 目的分析经阴道超声(TVS)与经腹部超声(TAS)在卵巢病变良、恶性病变鉴别诊断中的应用价值。方法选定本院超声科2020年1月至2022年6月接诊的50例卵巢病变患者,均给予TVS、TAS检查,将病理活检穿刺检查结果作为本次研究金标准,比较TVS、TAS、TVS+TAS诊断准确度、灵敏度、特异度,Kappa检验TVS、TAS、TVS+TAS与金标准的一致性。比较良性、恶性卵巢病变组超声血流分级、超声血流参数、Finkler超声评分。结果TVS+TAS诊断准确度(92.0%)、灵敏度(90.0%)、特异度(93.3%)均高于TVS(46.0%、40.0%、50.0%)、TAS(58.0%、50.0%、63.3%),P<0.05,TVS、TAS与金标准的一致性均一般(Kappa值=0.583、0.577),TVS+TAS与金标准的一致性较好(Kappa值=0.728),P<0.05。恶性组Ⅰ级(0)、Ⅱ级率(10.0%)均低于良性组(66.7%、26.7%),恶性组Ⅲ级(70.0%)、Ⅳ级率(20.0%)均高于良性组(6.7%、0),(P<0.05)。恶性组收缩期峰值流速(Vps)、平均流速(Vm)、Finkler超声评分均高于良性组,恶性组搏动指数(PI)、阻力指数(RI)均低于良性组(P<0.05)。结论TVS+TAS可提高卵巢病变良、恶性鉴别诊断准确度、灵敏度以及特异度,同时提供血流分级、血流参数、Finkler超声评分等信息,为临床鉴别病变良、恶性提供更多参考依据。 Objective To analyze the application value of transvaginal ultrasonography(TVS) and transabdominal ultrasonography(TAS) in the differential diagnosis of benign and malignant ovarian lesions. Methods A total of 50 patients with ovarian lesions who were admitted to the Ultrasound Department of our hospital from January 2020 to June 2022 were selected, and they were all examined by TVS and TAS. The results of biopsy biopsy were used as the gold standard for this study. TVS, TAS and TVS+TAS diagnostic accuracy, sensitivity, specificity were compared, Kappa test were used to analyze TVS, TAS, TVS+TAS and the gold standard consistency. The ultrasound blood flow grading, ultrasound blood flow parameters and Finkler ultrasound score were compared between benign and malignant ovarian lesions. Results The diagnostic accuracy(92.0%), sensitivity(90.0%) and specificity(93.3%) of TVS+TAS were higher than those of TVS(46.0%, 40.0%, 50.0%) and TAS(58.0%, 50.0%, 63.3%), P<0.05, the agreement between TVS, TAS and the gold standard was general(Kappa value=0.583, 0.577), and the consistency between TVS+TAS and the gold standard was good(Kappa value=0.728), P<0.05. The rates of grade Ⅰ(0)and grade Ⅱ(10.0%) in the malignant group were lower than those in the benign group(66.7%, 26.7%), and the rates of gradeⅢ(70.0%) and grade Ⅳ(20.0%) in the malignant group were higher than those in the benign group( 6.7%,0), P<0.05. The Vps, Vm, and Finkler ultrasound scores of the malignant group were higher than those of the benign group, and the PI and RI of the malignant group were lower than those of the benign group(P<0.05). Conclusion TVS +TAS can improve the accuracy, sensitivity and specificity of differential diagnosis of benign and malignant ovarian lesions, and provide information such as blood flow grading, blood flow parameters, Finkler ultrasound score,etc., and provide more reference for clinical differentiation of benign and malignant lesions.
