酶联免疫吸附法检测EB病毒两种抗体(VCA-IgM,EA-IgG)在儿童不明原因发热诊断中的意义 被引量:2

摘要 目的:研究针对儿童不明原因发热EB病毒(EBV)的两种抗体VCA-IgM和EA-IgG的诊断价值。方法:选择2020年2月~2021年2月本院收治的63例不明原因发热患儿进行研究,另选择同期幼儿园健康体检中的60例正常儿童作为对照组,运用酶联免疫吸附法(ELISA)检测患儿血清样本中的EBV抗体。结果:(1)63例患儿的EA-IgG抗体均显示为阴性。4例传染性单核细胞增多症的患儿VCA-IgM抗体均显示为阳性,其他均为阴性;(2)传染性单核细胞增多症组患儿的异淋比例、转氨酶活性水平升高幅度明显高于对照组、上呼吸道感染组、其他疾病组,组间差异显著(P<0.05);急性肝炎组转氨酶活性水平明显高于其他四组,组间差异显著(P<0.05);(3)传染性单核细胞增多症组、上呼吸道感染组、急性肝炎组和其他疾病组的白细胞、血红蛋白、血小板、EOS水平与对照组相近,组间比较无显著差异(P>0.05);各组间比较差异也无统计学意义(P>0.05)。结论:EBV的两种抗体均能够动态反映出EB感染后的抗体水平,具有高度敏感性和特异性,且检测实操方便简单,可将其作为诊断儿童不明原因发热的重要诊断方式。 Objective: Study the diagnostic value of VCA-IgM and EA-IgG antibodies against Epstein Barr virus(EBV) in children with unexplained fever. Methods: 63 children with unexplained fever treated in our hospital from February 2020 to February 2021 were selected for the study, and 60 normal children in the kindergarten physical examination in the same period were selected as the control group. The EBV antibodies in the serum samples of children were detected by enzyme-linked immunosorbent assay(ELISA). Results:(1)The EA-IgG antibodies of 63 children were negative. The VCA-IgM antibodies of 4 children with infectious mononucleosis were positive, and the others were negative;(2)The proportion of heterogony and the level of transaminase activity in children with infectious mononucleosis were significantly higher than those in the control group, upper respiratory tract infection group and other disease groups(P<0.05);The level of transaminase activity in acute hepatitis group was significantly higher than that in the other four groups(P<0.05);(3)The levels of leukocyte, hemoglobin, platelet and EOS in infectious mononucleosis group, upper respiratory tract infection group, acute hepatitis group and other disease groups were similar to those in the control group, and there was no significant difference between the groups(P>0.05);There was no significant difference between groups(P>0.05). Conclusion: Both antibodies of EBV can dynamically reflect the antibody level after EB infection, with high sensitivity and specificity, and the detection is simple and convenient. It can be used as an important diagnostic method for the diagnosis of unexplained fever in children.
