摘要
目的探讨乌鲁木齐市痛风人群的身体质量指数(body mass index,BMI)和腰臀比(waist-to-hip ratio,WHR)与痛风患者发病的危险性之间的关系。方法随机选取2015—2019年在乌鲁木齐市某三甲医院就诊的516例男性痛风患者为痛风组,同医院男性健康体检者516例为对照组,检验并分析相关血液生化指标,测量血压、腰围、臀围,并计算BMI和WHR。应用Logistic回归模型分析超重、肥胖和WHR与痛风发病风险的关联性。检验水准α=0.05。结果痛风组的尿酸、葡萄糖、尿素氮、肌酐、三酰甘油、低密度脂蛋白、舒张压、体质量和腰围均高于对照组且差异有统计学意义(P<0.05);两组的年龄、身高、收缩压差异均无统计学意义(P>0.05)。BMI和WHR与痛风的发生呈正相关关系(r=0.272、0.345,P<0.05);痛风组与对照组的BMI、WHR、腰围的构成比差异均有统计学意义(χ^(2)=55.338、54.928、54.153,P<0.05)。在调整年龄、有氧运动等多种混杂因素后,多因素非条件Logistic回归分析结果显示,BMI为24.00~27.99 kg/m^(2)、≥28.00 kg/m^(2)患者发生痛风OR值分别是BMI正常者的2.005(1.337,3.006)倍和2.677(1.668,4.296)倍;WHR为≥0.90的患者发生痛风的OR值是WHR正常者的1.668(1.186,2.345)倍且差异有统计学意义(P<0.05),年龄亚组分析结果与其相似。结论乌鲁木齐市痛风人群的BMI和WHR高于正常人群;BMI、腰围和WHR均与痛风的患病有关联。随着BMI和WHR上升发生痛风的危险性逐渐增高。
Objective To explore the relationship between body mass index(BMI),waist-hip ratio(WHR)and the risk of gout in Urumqi.MethodsA total of 516 male patients with gout in a third-class hospital in Urumqi from 2015 to 2019were randomly selected as the gout group and 516 male healthy subjects in the same hospital as the control group.The relevant blood biochemical indexes were examined and analyzed.Blood pressure,waist circumference and hip circumference were measured.Body mass index and waist-to-hip ratio were calculated.Logistic regression model was used to analyze the relationship between overweight/obesity,waist-to-hip ratio and the risk of gout.The test level isα=0.05.ResultsUric acid,glucose,urea nitrogen,creatinine,triglyceride,low-density lipoprotein,systolic blood pressure,weight and waist circumference in gout group were higher than those in control group,and the differences were statistically significant(P<0.05);There were no significant differences in age,height and diastolic blood pressure between the two groups(P<0.05).There was a positive correlation between BMI and WHR and the occurrence of gout(r=0.272,0.345,P<0.05).There were significant differences in BMI,WHR and waist circumference between the gout group and the control group(χ^(2)=55.338,54.928,54.153,P<0.05).After adjusting for age,aerobic exercise and other confounding factors,the results of multi-factor unconditional Logistic regression analysis showed that the odds ratio(OR)of gout in patients with BMI of 24.00-27.99 kg/m^(2)and≥28.00kg/m^(2)was 2.005(1.337-3.006)and 2.677(1.668-4.296)times higher than that of patients with normal BMI,respectively.The OR value of gout in patients with WHR≥0.90 was 1.668 times higher than that in patients with normal WHR,and the difference was statistically significant.The results of subgroup analysis according to age are generally similar.ConclusionsThe BMI and WHR of man with gout in Urumqi are higher than those of normal people,and BMI,waist circumference and WHR are all associated with the incidence of gout.The risk of gout increases with the increase of BMI and WHR.
