摘要
目的:观察小儿消积止咳口服液联合阿奇霉素治疗儿童支原体肺炎(MMP)的临床疗效。方法:将91例痰热壅肺型MMP患儿随机分为研究组46例与对照组45例。对照组给予阿奇霉素治疗,研究组在对照组的基础上加用小儿消积止咳口服液治疗。2组均治疗14 d。比较2组治疗前后25%肺活量时最大呼气流量(MEF_(25))、50%肺活量时最大呼气流量(MEF_(50))、最大呼气中段流量(MMEF_(25~75))、第1秒用力呼气量/用力肺活量(FEV_(1)/FVC)等肺功能指标,C-反应蛋白(CRP),白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α),降钙素原(PCT)等炎症因子水平,以及2组总体临床疗效、不良反应发生情况。结果:研究组治疗总有效率为93.48%,对照组为77.78%,2组比较,差异有统计学意义(P<0.05)。治疗前,2组MEF_(25)、MEF_(50)、MMEF_(25~75)、FEV_(1)/FVC比较,差异无统计学意义(P>0.05);治疗后,2组上述各项肺功能指标均较治疗前改善(P<0.05),且研究组各项指标改善较对照组更显著(P<0.05)。治疗前,2组CRP、IL-6、TNF-α、PCT水平比较,差异均无统计学意义(P>0.05);治疗后,2组上述各项指标均较治疗前降低(P<0.05),且研究组各项指标均低于对照组(P<0.05)。研究组不良反应发生率为17.39%,对照组为15.56%,2组比较,差异无统计学意义(P>0.05)。结论:小儿消积止咳口服液联合阿奇霉素治疗MMP疗效确切且安全性较高,可减轻患儿气道炎症,改善肺功能。
Objective:To observe the clinical effect of Xiao’er Xiaoji Zhike Oral Liquid combined with azithromycin for mycoplasma pneumonia(MMP) in children.Methods:A total of 91 cases of MMP children of phlegm-heat obstructing the lung type were randomly divided into the study group and the control group,with 46 and 45 cases in each group respectively.The control group was treated with azithromycin,and the study group was treated with Xiao’er Xiaoji Zhike Oral Liquid combined with azithromycin.Both groups were treated for 14 days.Before and after treatment,the maximum expiratory flow at 25%vital capacity(MEF_(25)),the maximum expiratory flow at 50%vital capacity(MEF_(50)),the maximum middle expiratory flow(MMEF_(25~75)),the first second forced expiratory volume/forced vital capacity(FEV_(1)/FVC) and other lung function indexes,as well as the levels of C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor(TNF-α),procalcitonin(PCT) and other inflammatory factors were compared between the two groups.The total clinical effects and adverse reactions in the two groups were compared.Results:The total effective rate was 93.48%in the study group and 77.78%in the control group,the difference being significant(P<0.05).Before treatment,there was no significant difference being found in the comparison of MEF_(25),MEF_(50),MMEF_(25~75),and FEV_(1)/FVC between the two groups(P>0.05).After treatment,MEF_(25),MEF_(50),MMEF_(25~75),and FEV_(1)/FVC in the two groups were improved when compared with those before treatment(P<0.05),and the improvement of each index in the study group was more significant than that in the control group(P<0.05).Before treatment,there was no significant difference being found in the comparison of levels of CRP,IL-6,TNF-α,and PCT between the two groups(P>0.05).After treatment,levels of CRP,IL-6,TNF-α,and PCT in the two groups were decreased when compared with those before treatment(P<0.05),and each index in the study group was lower than that in the control group(P<0.05).The incidence of adverse reactions was 17.39%in the study group and 15.56%in the control group,there being no significance in the difference(P>0.05).Conclusion:Xiao’er Xiaoji Zhike Oral Liquid combined with azithromycin has a definite curative effect and high safety in treating MMP,which can relieve airway inflammation and improve lung function in children.
