摘要
目的:分析多层螺旋CT(multi-slice spiral CT,MSCT)增强扫描在进展期胃癌患者中的诊断效能。方法:选取2021年6月—2022年6月东阿县人民医院收治的84例进展期胃癌患者为研究对象,对所有患者均行MSCT增强扫描,以病理检查结果为“金标准”,分析MSCT增强扫描在进展期胃癌患者中的诊断效能。结果:84例进展期胃癌患者中,病理检查证实穿透浆膜者共51例,未穿透浆膜者33例。MSCT局部浆膜面征象对癌肿穿透浆膜的诊断灵敏度92.16%(47/51)、准确率80.95%(68/84)高于脂肪间隙征象的50.98%(26/51)、49.38%(40/81)与癌肿局部突出征象检查的33.33%(17/51)、59.52%(50/84),差异有统计学意义(P<0.05)。84例进展期胃癌患者中,病理检查证实胰腺受侵35例,未受侵49例;横结膜与系膜受侵27例,未受侵57例;脾受侵12例,未受侵72例。MSCT增强扫描检查胰腺受侵、横结膜与系膜受侵犯以及脾受侵的特异度处于较高水平。结论:MSCT增强扫描在进展期胃癌的诊断中效能显著,能够精准评估病灶侵袭与浸润情况,从而为临床提供全面的参考信息。
Objective To analyze the diagnostic efficacy of multi-slice spiral computed tomography(MSCT)with enhanced scanning in patients with advanced gastric cancer.Methods A total of 84 patients with advanced gastric cancer admitted to Dong'e County People's Hospital from June 2021 to June 2022 were selected as the research objects.All patients underwent enhanced MSCT scan and pathological examination.The diagnostic efficacy of enhanced MSCT scan in patients with advanced gastric cancer was analyzed with pathological examination results as the"gold standard".Results Among 84 patients with advanced gastric cancer,there were 51 cases with serous membrane penetration and 33 cases without serous membrane penetration.The sensitivity 92.16%(47/51)and accuracy 80.95%(68/84)of local serous surface signs of MSCT in the diagnosis of cancer penetrating serous membrane were higher than that of adipose space signs 50.98%(26/51),49.38%(40/81)and local prominent signs of cancer 33.33%(17/51),59.52%(50/84),the difference was statistically significant(P<0.05).Among the 84 patients with advanced gastric cancer,35 cases were confirmed to be invaded by pathological examination,while 49 cases were not.There were 27 cases of transverse conjunctival and mesangial invasion and 57 cases of non-invasion.The spleen was invaded in 12 cases and not invaded in 72 cases.The specificity of pancreatic invasion,transverse conjunctival and mesangial invasion,and splenic invasion by MSCT enhancement scan was at a high level.Conclusion Enhanced MSCT scan has significant efficacy in the diagnosis of advanced gastric cancer,which can accurately evaluate the invasion and invasion of the lesion,so as to provide comprehensive reference information for clinical.
