摘要
介绍郑燕林教授从郁辨治中心性浆液性脉络膜视网膜病变的临床经验。郑燕林教授认为中心性浆液性脉络膜视网膜病变是由肝脾等脏腑功能失调,气血津液运行失常,络郁水停,水湿停聚,郁于视衣黄斑而成。病位在水轮视衣黄斑,与肝脾肾关系密切。湿郁黄斑为基本病机,湿郁贯穿疾病始终。故以开郁导滞、通络祛湿为基本治法,并在基本治法的基础上辨证施治,灵活运用疏肝健脾、通络祛湿、利湿清热或清热化痰、温阳利水、滋阴利水等治法。
This paper introduces the clinical experience of Professor ZHENG Yanlin in treating central serous chorioretinopathy from syndrome differentiation and treatment of depression.Professor ZHENG holds that central serous chorioretinopathy is caused by the dysfunction of the liver,spleen and other viscera,the abnormal movement of qi,blood,fluid,essence and spirit,the collateral depression and water retention,the water-dampness retention,and the depression in retina and macula lutea.The disease location lies in the water orbiculus,retina,and macula lutea,which is closely related to the liver,spleen and kidney.Dampness depression in macula lutea is the basic pathogenesis,and dampness depression exists through the whole process of the disease.Therefore,the basic treatment method is to resolve depression and stagnation,and to unblock collaterals and eliminate dampness,and based on which the syndrome differentiation and treatment is carried out,and various methods are flexibly applied,including smoothing the liver and fortifying the spleen,unblocking collaterals and eliminating dampness,draining dampness and clearing heat or clearing heat and resolving phlegm,warming yang and promoting urination,and enriching yin and promoting urination.
