改良迷你体外循环系统在体重小于55 kg成人中应用的临床效果及对炎性因子的影响

摘要 目的评价阜外改良迷你体外循环(FUWAI-SAVE)系统在体重<55 kg成人患者中的临床效果及其对炎性因子的影响。方法选取2020年6月至2021年7月在本院行择期体外循环下心脏外科手术体重<55 kg的成人患者60例,随机分为对照组和FW-SAVE组进行前瞻性研究。收集患者一般情况、转机信息、血液制品使用情况、临床结局以及不同阶段炎性因子表达情况。结果对照组和FW-SAVE组患者一般情况,转机情况差异无统计学意义。FW-SAVE组红细胞输入量低于对照组[0(0,0)vs.0(0,1.5),Z=-2.380,P=0.026],两组间血浆及血小板使用量差异无统计学意义。临床结局中FW-SAVE组与对照组均未发生院内死亡,其中两组间并发症出现率为[(0/30)vs.(2/30),χ2=2.069,P=0.15],差异无统计学意义。两组间各阶段炎性因子差异均无统计学意义。结论FUWAI-SAVE系统在体重<55 kg的成人患者中的应用具有较好的临床效果,并能够减少临床血液制品的使用。 Objective To evaluate the clinical effect of FUWAI-SAVE system in adults less than 55 kg and its effect on inflammatory factors.Methods From June 2020 to July 2021,60 patients less than 55 kg who underwent cardiac surgery with elective cardiopulmonary bypass(CPB)in our hospital were selected and randomly divided into control group and FW-SAVE group for the prospective study.The general condition of patients,information of CPB,blood product usage,clinical outcomes,and the expression of inflammatory factors at different stage were collected.Results There was no statistically significant difference between the control group and the FW-SAVE group in terms of general conditions and CPB.The red blood cell transfusion in the FW-SAVE group was lower than that in the control group[0(0,0)vs.0(0,1.5),Z=-2.380,P=0.026].There was no significant difference in the transfusion of plasma and platelets between the two groups.There was no in-hospital death in both groups,and the incidence of complications between the two groups was[(0/30)vs.(2/30),χ~2=2.069,P=0.15].There was no significant difference in inflammatory factors at all stages between the two groups.Conclusion The application of FUWAI-SAVE system in adults less than 55 kg has good clinical effect and can reduce the use of clinical blood products.
