HIF-1α在甲状腺乳头状癌中的表达及临床特征的相关性分析 被引量:1

摘要 目的 研究缺氧诱导因子1α(HIF-1α)在甲状腺乳头状癌(PTC)中的表达,并分析其表达与临床特征及超声特征的相关性。方法 采用回顾性研究,收集2019年1月至6月首都医科大学附属北京友谊医院收治的59例PTC患者的临床病理及术前超声资料,将癌旁组织作为对照组,采用免疫组织化学检测其HIF-1α表达水平,比较两组间HIF-1α表达,同时纳入多个临床病理参数,分析PTC中HIF-1α表达与临床特征及超声特征的相关性。结果 PTC中HIF-1α的表达阳性率为52.5%,显著高于癌旁组织(3.4%),差异有统计学意义(P<0.05)。有包膜浸润PTC中HIF-1α表达阳性率为65.9%,高于无包膜浸润(33.3%),差异有统计学意义(P<0.05);有淋巴结转移与无淋巴结转移的HIF-1α表达差异无统计学意义(P>0.05);在性别、年龄、肿瘤大小及超声特征(如回声、边界、钙化、纵横比、声晕等)等方面HIF-1α表达差异无统计学意义(P>0.05)。结论 HIF-1α与PTC发生相关,其高表达可能提示PTC早期发生包膜浸润,可作为早期监测PTC侵袭行为的生物学指标。 Objective To investigate the expression of hypoxia-induced factor 1α(HIF-1α) in papillary thyroid carcinoma(PTC) and evaluate their possible relationships with clinical behavior and ultrasound features. Methods Using a retrospective study, the clinicopathological and preoperative ultrasound data of 59 patients with papillary thyroid carcinoma admitted to Beijing Friendship Hospital, Capital Medical University from January to June 2019 were collected. Immunohistochemistry(IHC) was performed to detect the expression level of HIF-1α, and para-cancerous tissues as control group. Furthermore, various parameters were assessed to determine their correlations with HIF-1α expression. Results The positive expression rate of HIF-1α in PTC tissues was 52.5%, which was significantly higher than and that in normal tissues(3.4%) by IHC, and the difference was statistically significant(P<0.05). The positive expression rate of HIF-1α in PCT with capsule infiltration was 65.9%,which was higher than that without capsule infiltration(33.3%), and the difference was statistically significant(P<0.05);there was no statistical difference of HIF-1α expression between the lymph node metastasis group and non-lymph node metastasis group(P>0.05). The expression of HIF-1α showed no significant difference in PTC patients with gender, age distribution, tumor size and ultrasound features(such as Echo, boundary, calcification, aspect ratio, sound halo)(P>0.05). Conclusion Overexpression of HIF-1α is associated with carcinogenesis of PTC and capsule invasion in PTC. It could be used as a biological indicator to monitor the aggressive behavior of PTC in early stage.
