摘要
目的 观察sysmex系列全自动血细胞分析仪散点图,探讨白细胞分类散点图在白细胞计数(WBC)正常的急性白血病患者筛查中的诊断价值。方法 选择2013年9月28日—2022年3月31日在曲靖市第二人民医院就诊并首次诊断为急性白血病的129例患者作为急性白血病组,选择同期100名健康体检者作为对照组。比较两组受检者的血常规指标,统计急性白血病组患者的血细胞计数和血细胞形态异常特征,通过全自动血细胞分析仪获取患者的白细胞散点图,观察白细胞散点图各区域的散点变化。结果 急性白血病组患者的WBC明显高于对照组,血红蛋白(Hb)及血小板计数(PLT)均明显低于对照组[WBC(×10^(9)/L):35.87±6.22比6.05±1.28,Hb(g/L):80.83±27.95比151.80±12.55,PLT(×10^(9)/L):71.42±9.38比243.34±19.69,均P<0.05],129例患者中白细胞升高61例,降低39例,异常率为77.52%(100/129);Hb水平升高4例,降低110例,异常率为88.37%(114/129);PLT水平升高2例,降低103例,异常率为81.40%(105/129)。20例WBC正常的患者中,2例为急性淋巴细胞白血病,其余18例均为急性髓系白血病(其中2例为急性早幼粒细胞白血病)。结论 对WBC正常的急性白血病患者,结合白细胞分类散点图能更早获得诊断。
Objective To observe the scatter diagram of sysmex series automatic blood cell analyzer, and to explore the diagnostic value of leukocyte classification scatter diagram in the screening of acute leukemia patients with normal white blood cell count(WBC). Methods A total of 129 patients who were first diagnosed with acute leukemia in the Second People’s Hospital of Qujing from September 28, 2013 to March 31, 2022 were selected as the acute leukemia group, and 100 healthy subjects undergoing physical examination in the hospital during the same period were selected as the control group. The blood routine indexes of the two groups were compared, and the abnormal blood cell count and morphology of patients in the acute leukemia group were counted. The white blood cell scatter map of patients was obtained by automatic blood cell analyzer, and the scatter changes of each area on the white blood cell scatter map were observed. Results The level of WBC in acute leukemia group was higher than that in control group, and the levels of hemoglobin(Hb) and platelet count(PLT) were lower than those in the control group [WBC(×10^(9)/L): 35.87±6.22 vs. 6.05±1.28, Hb(g/L): 80.83±27.95 vs. 151.80±12.55, PLT(×10^(9)/L): 71.42±9.38 vs. 243.34±19.69, all P < 0.05]. Among the 129 patients, the level of WBC increased in 61 cases and decreased in 39 cases, and the abnormal rate was 77.52%(100/129). The level of Hb increased in 4 cases and decreased in 110 cases, with an abnormal rate of 88.37%(114/129). The level of PLT increased in 2 cases and decreased in 103 cases, with an abnormal rate of 81.40%(105/129). Two of the 20 patients with normal WBC level were acute lymphoblastic leukemia, and the remaining 18 patients were all with acute myeloid leukemia(including two cases of acute promyelocytic leukemia). Conclusion Acute leukemia could be detected earlier in patients with normal WBC level by combining leukocyte classification scatter map.
