摘要
目的 探讨江苏省35~75岁成人心率水平与正常高值血压、高血压的关系,为高血压防治提供科学依据。方法 利用2015—2019年江苏省8个项目点开展的心血管病高危人群筛查项目资料,共140 281名对象纳入本研究。将心率分为3个水平(<70、70~80、>80次/min),用logistic回归分析探讨不同心率水平与正常高值血压、高血压的关系。结果 目标人群正常高值血压、高血压检出率分别为30.75%和57.76%,心率<70、70~80、>80次/min人群分别占31.95%、39.20%、28.85%,正常高值血压人群中3个不同心率水平分别占31.65%、41.21%、27.14%;高血压人群中分别占31.70%、37.28%、31.02%。多因素logistic回归分析显示,以心率<70次/min为参照,心率70~80次/min、>80次/min者正常高值血压风险的OR值分别为1.153(95%CI为1.103~1.204)、1.589(95%CI为1.509~1.672),高血压风险OR值分别为1.181(95%CI为1.129~1.236)、2.141(95%CI为2.031~2.256)。结论 正常高值血压、高血压检出风险随心率加快而增加,在进行血压控制的同时应加强心率的管理。
Objective To examine the associations of heart rate with normal-high blood pressure and hypertension among adults age 35 to 75 years;to provide insights into hypertension control. Methods A total of 140 281 subjects from 8 pilot sites in Jiangsu Province from 2015 to 2019 for screening individuals at a high risk of cardiovascular diseases were enrolled, and assigned into three groups(<70,70 to 80 and > 80 beats/min) according to heart rate. The associations of heart rate with normal-high blood pressure and hypertension were examined among adults using the logistic regression analysis. Results The prevalence of high-normal blood pressure and hypertension was 30.75% and 57.76% among the participants, among whom 31.95% had a heart rate of<70 beats/min, 39.20% with a heart rate of 70 to 80 beats/min, and 28.85% with a heart rate of > 80 beats/min. Among the participants with high-normal blood pressure, there were 31.65% with a heart rate of<70 beats/min, 41.21% with a heart rate of 70 to 80 beats/min, and 27.14% with a heart rate of > 80 beats/min;and among the hypertensive participants, there were 31.70% with a heart rate of<70 beats/min, 37.28% with a heart rate of 70 to 80 beats/min, and 31.02% with a heart rate of > 80 beats/min. Multivariate regression analysis showed that the participants with a heart rate of 70 to 80 [OR=1.153,95%CI:(1.103,1.204)] and > 80 beats/min [OR = 1.589,95%CI:(1.509,1.672)] were more likely to develop normal-high blood pressure and hypertension [OR=1.181,95%CI:(1.129,1.236);OR = 2.141,95%CI:(2.031,2.256)] relative to those with a heart rate of<70 beats/min. Conclusion The risk of normal-high blood pressure and hypertension increases with heart rate, and the management of heart rate requires to be improved in addition to blood pressure control.
