摘要
目的探讨益气补血利水渗湿法治疗中风后尿潴留的临床效果。方法选取2017年1月至2021年9月河北省承德市中医院收治的122例中风后尿潴留患者,按照随机数字表法分成参照组和观察组,各61例。参照组给予基础治疗和膀胱功能训练,观察组在参照组基础上给予益气补血利水渗湿法综合治疗,两组均治疗14 d。比较两组临床疗效、中医证候评分、排尿情况、膀胱功能评分和特定脑卒中生活质量(SS-QOL)评分。结果观察组临床疗效优于参照组,差异有统计学意义(P<0.05)。治疗后两组少腹胀满、排尿无力、神疲乏力评分均较治疗前降低,且观察组低于参照组,差异有统计学意义(P<0.05)。治疗后两组排尿次数、平均尿量均较治疗前增多,残余尿量较治疗前减少,且观察组排尿次数、平均尿量均多于参照组,残余尿量少于参照组,差异有统计学意义(P<0.05)。治疗后两组膀胱功能评分较治疗前降低,SS-QOL评分较治疗前升高,且观察组膀胱功能评分低于参照组,SS-QOL评分高于参照组,差异有统计学意义(P<0.05)。结论益气补血利水渗湿法综合治疗中风后尿潴留的效果显著,可明显改善膀胱功能。
Objective To investigate clinical effect of supporting qi and enriching blood and promoting diuresis and diffusing dampness method in the treatment of urinary retention after stroke.Methods One hundred and twenty-two patients with urinary retention after stroke admitted to Chengde Traditional Chinese Medicine Hospital,Hebei Province from January 2017 to September 2021 were selected and divided into reference group and observation group according to the random number table method,with 61 cases in each group.Reference group was given basic treatment and bladder function training,and observation group was given comprehensive treatment of supporting qi and enriching blood and promoting diuresis and diffusing dampness method on the basis of reference group.Both groups were treated for 14 days.Clinical effect,traditional Chinese medicine syndrome score,urination,bladder function score and specific stroke quality of life(SS-QOL)score between the two groups were compared.Results Clinical effect of observation group was better than that of reference group,the difference was statistically significant(P<0.05).After treatment,the scores of hypoabdominal distension,micturition weakness and mental fatigue in the two groups were lower than those before treatment,and observation group was lower than that control group,the differences were statistically significant(P<0.05).After treatment,the number of urination and average urine volume in the two groups were more than those before treatment,and the residual urine volume in the two groups was less than that before treatment,and the number of urination and average urine volume in observation group were more than those in reference group,and the residual urine volume in observation group was less than that in reference group,the differences were statistically significant(P<0.05).After treatment,the bladder function score of the two groups was lower than that before treatment,and the SS-QOL score of the two groups was higher than that before treatment,and the bladder function score of observation group was lower than that of reference group,and the SS-QOL score of observation group was higher than that of reference group,the differences were statistically significant(P<0.05).Conclusion The comprehensive treatment of supporting qi and enriching blood and promoting diuresis and diffusing dampness method in the treatment of urinary retention after stroke has a significant effect,and it can significantly improve bladder function.
