摘要
目的探讨中国侵袭性曲霉病(IA)患者的诊疗情况及经济负担。方法通过中国期刊全文数据库、中文期刊全文数据库、万方数据知识服务平台、PubMed以及Embase数据库检索截至2020年12月10日使用中文及英文发表的研究文献,根据纳入排除标准筛选文献后,收集有关IA流行病学特征、经济负担、生命质量以及临床治疗相关数据。在文献综述基础上,根据目前缺失的研究数据设计问卷,对我国6个城市,19家医院的25位医生进行调研,收集IA患者在门诊和住院期间的所有花费,从全社会角度出发,基于Microsoft Excel 2019建模后计算出IA患者人均经济负担。结果文献检索共纳入26篇文献,综述结果显示,IA疾病多发于中老年人群,至诊断为IA时平均年龄为43~77岁,男性患者比例(69.3%)高于女性。专家调研中共收集到24份IA有效问卷,结果显示,IA患者人均经济负担为113844元。在人均经济负担中,药品费用占比最高(74.68%),为76805元。单因素敏感性分析显示,影响IA患者人均经济负担的前4位关键参数分别是总用药天数、日均药品费用、检查检验费用和首次住院时间。结论中国IA患者经济负担较重,政府和医疗机构应引起重视并采取合理的政策和措施降低IA患者的经济负担,其中,减轻用药成本极为重要,鼓励给药途径简单、疗效更优、安全性更高的抗IA药物的开发和使用。
Objective To explore the diagnosis and treatment of invasive aspergillosis(IA)patients in China and their economic burden.Methods The research literature published in Chinese and English as of December 10,2020 was retrieved through the Chinese Journal Full Text Database,Chinese Journal Full Text Database,Wanfang Data Knowledge Service Platform,PubMed and Embase databases.After screening the literature according to the inclusion exclusion criteria,relevant data on IA epidemiological characteristics,economic burden,quality of life and clinical treatment were collected.On the basis of literature review,a questionnaire was designed according to the current missing research data,and 25 doctors from 19 hospitals in 6 cities in China were investigated.All the expenses incurred by IA patients during outpatient and inpatient periods were collected.From the perspective of the whole society,the per capita economic burden of IA patients was calculated based on Microsoft Excel 2019 modeling.Results A total of 26 publications were included in the literature search,and the results of the retrospective cohort study showed that IA disease is more prevalent in the middle-aged and elderly population,with a mean age of 43~77 years by the time of diagnosis of IA,and more male patients(69.3%)than female patients.A total of 24 valid questionnaires for IA were collected in the expert study.The total economic burden per capita for IA patients was 113844.Among the total financial burden per capita,drug costs accounted for the highest percentage(74.68%),at 76805.One way sensitivity analysis showed that the top four key parameters affecting medical costs for IA patients were total drug days,average daily drug costs,examination and test costs and duration of first hospitalization.Conclusion Antifungal therapy imposing significant economic burden in IA patients in China.This study highlights the critical need to introduce innovative therapies with better clinical efficacies,longer survival,reduced total cost and a simple admission for IA patients in China.
