摘要
目的:探究组合式呼吸训练联合振动排痰对老年稳定期支气管扩张症患者排痰效果及肺功能恢复的影响。方法:选择本院2019年12月至2021年12月收治的50例老年稳定期支气管扩张症患者,按照随机数字表法分为对照组(n=25)和观察组(n=25)。对照组采取组合式呼吸训练,观察组于此基础上运用振动排痰,均持续干预2周。比较两组排痰效果、血气指标及肺功能。结果:干预前,两组日排痰量、痰液黏稠度、血气指标、肺功能比较,差异无统计学意义(P>0.05);干预后,观察组日排痰量多于对照组,痰液黏稠度评分低于对照组,动脉血氧(PaO 2)高于对照组,二氧化碳分压(PaCO 2)低于对照组,第1秒用力呼气量(FEV 1)、最大呼气峰流速(PEF)及FEV 1/用力肺活量(FVC)均高于对照组,差异有统计学意义(P<0.05)。结论:组合式呼吸训练联合振动排痰能够有效增加老年稳定期支气管扩张症患者日排痰量,降低痰液黏稠度,调节血气指标,促进肺功能改善。
Objective:To explore the effect of combined breathing training combined with vibration expectoration on expectoration and pulmonary function recovery in elderly patients with stable bronchiectasis.Methods:50 elderly patients with stable bronchiectasis treated in our hospital from December 2019 to December 2021 were randomly divided into control group(n=25)and observation group(n=25).The control group took combined breathing training,and the observation group used vibration sputum excretion on this basis,which lasted for 2 weeks.The expectoration effect,blood gas index and pulmonary function were compared between the two groups.Results:Before intervention,there was no significant difference in daily sputum output,sputum viscosity,blood gas index and lung function between the two groups(P>0.05);after the intervention,the daily sputum output of the observation group was more than that of the control group,the sputum viscosity score was lower than that of the control group,the arterial blood oxygen(PaO 2)was higher than that of the control group,the partial pressure of carbon dioxide(PaCO 2)was lower than that of the control group,and the first second forced expiratory volume(FEV 1),maximum peak expiratory flow rate(PEF)and FEV1/forced vital capacity(FVC)were higher than those of the control group(P<0.05).Conclusion:Combined breathing training combined with vibration sputum excretion can effectively increase the daily sputum excretion of elderly patients with stable bronchiectasis,reduce the viscosity of sputum,regulate blood gas indexes and promote the improvement of pulmonary function.
