摘要
目的 分析罗哌卡因椎管内麻醉复合舒芬太尼对初产妇分娩镇痛效果及产程的影响。方法 选择医院2019年2月-2020年2月收治的初产妇83例。根据麻醉方案不同分为常规组(n=40)和联合组(n=43)。常规组进行常规椎管内麻醉,联合组进行罗哌卡因椎管内麻醉复合舒芬太尼麻醉。比较两组一般指标、镇痛、新生儿情况和不良反应。结果 联合组产程活跃期、第二产程、产后出血量少于常规组(P<0.05),第三产程时间与对照组比较差异无统计学意义(P>0.05)。随着麻醉时间延长,两组初产妇VAS评分呈下降趋势,其中联合组初产妇注药10min、注药60min、注药120min VAS评分低于常规组(P<0.05)。随着新生儿出生时间延长,出生1min和5min Apgar评分增加;但两组新生儿1min和5min Apgar评分差异比较无统计学意义(P>0.05)。联合组不良反应率20.93%与常规组17.5%比较差异无统计学意义(P>0.05)。结论 罗哌卡因椎管内麻醉复合舒芬太尼麻醉初产妇,能显著减少分娩疼痛,缩短第1和第2产程时间,减少出血量,建议使用。
Objective To investigate the effect of intraspinal anesthesia with sufentanil and ropivacaine on labor analgesic effects and stages of labor of primiparas.Methods A total of 83 primiparas in our hospital from February 2019 to February 2020 were enrolled, and divided into two groups according to different anesthesia methods.Routine group(n=40)received traditional intraspinal anesthesia, while combined group(n=43)received intraspinal anesthesia with sufentanil and ropivacaine.Then the general indexes, analgesic effect, Apgar score and adverse reaction rate were compared between groups.Results The active stage of labor, the second stage of labor and the amount of postpartum hemorrhage in combined group were significantly less than those in routine group(P<0.05),and no significant difference was found in the time of third stage of labor between groups(P>0.05).With the prolonged anesthesia time, VAS scores had a decreasing trend in both groups.VAS scores at the10min, 60min and 120min after infusion were significantly lower in combined group than in routine group(P<0.05).Apgar score at 1min and 5min after birth increased in both groups, and showed no significant difference between groups(P>0.05).The adverse reaction rate was 20.39% in combined group, which had no significant difference with 17.5% in routine group(P>0.05).Conclusion Application of intraspinal anesthesia with sufentanil and ropivacaine can effectively alleviate the labor pain, shorten the first and second stages of labor, and reduce the amount of blood loss, which is worthy of recommendation.
