失效模式与效应分析在预防CT增强检查碘对比剂不良反应中的应用 被引量:4

Application of failure mode and effect analysis in the prevention of adverse reactions of iodine contrast agent in enhanced CT examination

摘要 目的探讨失效模式与效应分析在预防碘对比剂不良反应中的应用效果。方法选取2020年1—12月于医院影像中心CT增强检查的31922例患者作为对照组;选取2021年1—12月CT增强检查的32101例患者作为观察组。对照组采用常规护理,观察组运用失效模式与效应分析,对CT增强检查碘对比剂不良反应发生的影响因素列出可能的失效模式,制订改进方案,计算干预前后患者失效模式风险优先系数(RPN),并调查干预前后患者对CT增强检查碘对比剂运用的认知水平、完全水化率、碘对比剂不良反应的发生率及患者满意率。结果观察组检查前健康宣教不足、用药注意事项交待不足、各区域宣教设施不足、健康宣教方式不当、信息化程度不足的RPN低于对照组(P<0.05);观察组对碘对比剂运用的认知水平评分高于对照组(P<0.05);观察组患者碘对比剂不良反应发生率低于对照组、满意率高于对照组,差异均有统计学意义(P<0.05)。结论失效模式和效应分析应用于预防碘对比剂不良反应的护理干预中,可提高患者执行检查注意事项的依从性,进而降低碘对比剂不良反应的发生率,提升患者对影像检查服务的满意率,改善就医体验。 Objective To explore the application effect of failure mode and effect analysis in the prevention of adverse reactions of iodine contrast agent.Methods To select 31,922 patients who underwent enhanced CT examination in the imaging center of hospital from January to December 2020 as a control group,and 32,101 patients with enhanced CT from January to December 2021 were selected as an observation group.The control group used routine nursing,while the observation group used failure mode and effect analysis to list the possible failure modes for the influencing factors of adverse reactions of iodine contrast agent in enhanced CT examination,formulate an improvement plan,calculate the risk priority number(RPN)of the failure modes of patients before and after the intervention,and investigate the cognitive level of patients before and after the intervention on the use of iodine contrast agent in enhanced CT examination,the complete hydration rate,the incidence of adverse reactions of iodine contrast agent,and the patient satisfaction rate.Results The RPN of the observation group was lower than that of the control group in terms of insuffi cient health education before examination,insuffi cient disclosure of medication precautions,insuffi cient education facilities in various regions,improper health education methods,and insuffi cient information level(P<0.05).The cognitive level score of iodine contrast agent use in the observation group was higher than that in the control group(P<0.05).The incidence of adverse reactions of iodine contrast agent in the observation group was lower than that in the control group,the satisfaction rate was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion The application of failure mode and effect analysis in nursing intervention to prevent adverse reactions of iodine contrast agent can improve the compliance of patients to implement inspection precautions,thereby reducing the incidence of adverse reactions of iodine contrast agent,improving the satisfaction rate of patients with imaging examination services,and improving the medical experience.
