神经传导及F波测定对亚临床糖尿病周围神经病变的应用价值研究 被引量:4

摘要 目的探究分析神经传导及F波测定在亚临床糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)中的应用价值。方法选取2021年10月—2022年10月于江苏省睢宁县人民医院就诊的186例患者,将其按照是否合并2型糖尿病、是否有周围神经损害症状分为3组,即对照组(无周围神经损害症状的健康者)62名、亚临床DPN组(无周围神经病损害症状的糖尿病患者)62例、DPN组(有周围神经损害症状的2型糖尿病患者)62例。比较3组周围神经传导数值、F波数值以及亚临床DPN上下肢神经传导速度、运动神经传导速度(MCV)、感觉神经传导速度(SCV)检测异常情况。结果3组运动神经传导速度(MCV)、感觉神经传导速度(SCV)及F波最短潜伏期、平均潜伏期、异常率比较,差异有统计学意义(P<0.05)。亚临床DPN组下肢神经传导异常率为38.71%,高于上肢异常率(25.40%),SCV异常率为36.56%,高于MCV异常率(27.02%),差异有统计学意义(χ^(2)=8.782、4.517,P<0.05)。结论神经传导联合F波检测对亚临床糖尿病周围神经病变有较高应用价值,可为本病早期干预治疗提供有力参考。 Objective To explore and analyze the application value of nerve conduction and F-wave measurement in subclinical diabetes peripheral neuropathy(DPN).Methods 186 patients who visited Suining County People's Hospital in Jiangsu Province from October 2021 to October 2022 were selected and divided into three groups according to whether they had type 2 diabetes and whether they had peripheral nerve damage symptoms,namely,62 people in the control group(healthy people without peripheral nerve damage symptoms),62 cases in the subclinical DPN group(diabetes patients without peripheral nerve damage symptoms),and 62 cases in the DPN group(diabetes patients with peripheral nerve damage symptoms).The peripheral nerve conduction value,F-wave value and the abnormal detection of upper and lower limb nerve conduction velocity,motor nerve conduction velocity(MCV)and sensory nerve conduction velocity(SCV)in subclinical DPN were compared among the three groups.Results There were statistically significant differences in motor nerve conduction velocity(MCV),sensory nerve conduction velocity(SCV)and the shortest latency,average latency and abnormal rate of F-wave among the three groups,and the difference was statistically significant(P<0.05).The abnormal rate of lower limb nerve conduction in subclinical DPN group was 38.71%,higher than that of upper limb(25.40%),and the abnormal rate of SCV was 36.56%,higher than that of MCV(27.02%),and the difference was statistically significant(χ^(2)=8.782,4.517,P<0.05).Conclusion Nerve conduction combined with F-wave detection has a high application value for subclinical diabetes peripheral neuropathy,which can provide a strong reference for early intervention treatment of the disease.
