摘要
目的:探讨囊腔性肺癌的MSCT诊断价值。方法:回顾性分析本院18例,经病理诊断或临床证实的囊腔性肺癌的临床和MSCT资料,多平面重组(MPR)图像,有随访的病例,观察病灶的动态变化。结果:病灶位于双肺上叶13例,其中右侧7例、左侧6例,双肺下叶各2例,右肺中叶1例,单纯囊腔3例,囊腔伴结节状软组织密度6例,囊腔伴磨玻璃密度影5例,囊腔伴分隔4例,囊腔内可见血管穿行和支气管通入腔内5例,囊腔内可见支气管截断或闭塞4例,囊腔内可见血管穿行7例;3例随访病例囊腔消失,实成分明显增多,周围可见分叶和毛刺及胸膜凹陷征,2例表现囊腔扩大。结论:囊腔性肺癌较典型的CT表现征象为囊腔伴实性结节或磨玻璃密度,随访过程中囊壁增厚,实性或磨玻璃成分增大,可作出较准确的诊断。
Objective:To investigate the diagnostic value of multi-slice spiral CT(MSCT) in cystic lung cancer.Methods:It retrospectively analyzed the clinical and MSCT data of 18 cases of cystic lung cancer diagnosed by pathology or clinically confirmed in our hospital,multiplanar reconstituted MPR images and cases with follow-up,and observed the dynamic changes of the lesions.Results:The lesions were located in the upper lobes of both lungs in 13 cases,including 7 cases on the right side and 6 cases on the left side.The lesions were located in the lower lobes of both lungs,2 cases on the right and 2 cases on the left.The lesion was located in the middle lobe of the right lung in 1 case.In addition,there were 3 cases with simple cystic cavity,6 cases with cystic cavity with nodular soft tissue density,5 cases with cystic cavity with ground glass density shadow,4 cases with cystic cavity with segregation,5 cases with vascular penetration and bronchial passage into the cavity,4 cases with bronchial truncation or occlusion in the cavity,and 7 cases with vascular penetration in the cavity.In three cases with follow-up,the cystic cavity disappeared and the solid component increased significantly,and lobulation and burr and pleural depression signs were seen around it.Two cases showed enlargement of the cystic cavity.Conclusion:The more typical CT presentation signs of cystic lung cancer are cystic cavity with solid or ground glass densities with vascular penetration and separation within the cavity.There were dynamic changes in the lesions during the follow-up.The accurate diagnosis can be made accordingly.
