摘要
目的了解某三甲医院住院急性中毒患者临床特征,探讨影响预后的相关危险因素。方法回顾性分析2012年1月—2020年12月本院急诊部收治的急性中毒住院患者资料。结果460例住院中毒患者中,223例(48.48%)系药物中毒,是急性中毒需住院患者第一病因,农药中毒(142例,30%)位列第二,21例(80.77%)死亡患者系农药中毒,农药中毒是急性中毒住院患者的第一位死亡原因。自杀是急性中毒的最主要原因,占比80.43%;洗胃治疗比例由2012—2014年间的108例(67.08%)下降至2018—2020组的60例(43.17%)。影响急性中毒患者预后的独立危险因素为毒物种类(农药)(OR:8.765,95%CI:2.412-31.845,P<0.01)、机械通气(OR:5.285,95%CI:1.261-11.709,P<0.01)、血液净化治疗(OR:3.842,95%CI:1.533-18.221,P<0.05)。结论本市中心城区急性中毒发生率有下降趋势,药物中毒发生率最高,农药中毒是急性中毒患者的第一死亡原因,洗胃治疗比例呈现显著下降趋势。急性中毒住院患者预后受多种因素综合作用,导致死亡的独立危险因素是毒物种类(农药)、是否机械通气、是否血液净化治疗。
Objective To understand the clinical characteristics of inpatients with acute poisoning those treated in a tertiary hospital,and to explore the relevant risk factors affecting the prognosis.Methods The clinical data of inpatients with poisoning those treated in Zhongshan hospital affiliated to Xiamen University from January 2012 to December 2020 were retrospectively analyzed.Results Among 460 hospitalized patients with poisoning,223(48.48%)were drug poisoning,which was the first cause of acute poisoning requiring hospitalization.Pesticide poisoning(142,30%)ranked second,and 21(80.77%)died of pesticides,and pesticide poisoning was the first cause of death in hospitalized patients with acute poisoning.Suicide was the main cause of acute poisoning,accounting for 80.43%;the proportion of gastric lavage treatment decreased from 108 cases(67.08%)in 2012-2014 to 60 cases(43.17%)in 2018-2020 group.The independent risk factors affecting the prognosis of patients with acute poisoning were type of poison(pesticides)(OR:8.765,95%CI:2.412-31.845,P<0.01),mechanical ventilation(OR:5.285,95%CI:1.261-11.709,P<0.01),blood purification therapy(OR:3.842,95%CI:1.533-18.221,P<0.05).Conclusions The incidence of acute poisoning in the central urban area of Xiamen showed a downward trend,and the incidence of drug poisoning was the highest.Pesticide poisoning was the first cause of death in patients with acute poisoning,and the proportion of gastric lavage treatment showed a significant downward trend.The prognosis of hospitalized patients with acute poisoning is affected by a combination of factors.The independent risk factors for death are type of poisoning(pesticides),whether mechanical ventilation and blood purification are used..
