摘要
目的:探讨降钙素原(PCT)、B型脑钠肽(BNP)及肿瘤坏死因子-α(TNF-α)在脑卒中伴肺部感染患者中的应用价值。方法:回顾性分析2020年1—12月深圳市中西医结合医院收治的88例脑卒中伴肺部感染患者作为感染组,同期收集80例脑卒中未伴发肺部感染患者作为对照组,比较两组患者血清PCT、BNP、TNF-α水平,并分析其与神经功能缺损评分(NIHSS)的相关性。结果:感染组患者PCT、BNP、TNF-α水平明显高于对照组患者,差异有统计学意义(t=23.453、3.233、19.654,P<0.05);PCT、BNP、TNF-α联合诊断脑卒中伴肺部感染的曲线下面积(AUC)最大,灵敏度和特异度可达到90.0%、85.3%;PCT、BNP及TNF-α水平与NIHSS评分呈正相关,差异有统计学意义(r=0.303、0.361、0.154,P<0.05)。结论:血清PCT、BNP及TNF-α水平在脑卒中伴肺部感染患者中明显升高,可反映患者神经功能缺损情况,而且在脑卒中伴肺部感染的早期即可诊断。
Objective:To investigate the value of calcitoninogen(PCT),B-type brain natriuretic peptide(BNP),and tumor ne⁃crosis factor-α(TNF-α)in patients with stroke with pulmonary infection.Methods:88 patients with stroke with pulmonary infec⁃tion admitted to the hospital from January to December 2020 were retrospectively analyzed as the infection group,and 80 patients with stroke without pulmonary infection were collected as the control group during the same period.Serum PCT,BNP and TNF-αlevels were compared between the two groups,and their correlation with the neurological deficit score(NIHSS)was analyzed.Re⁃sults:The levels of PCT,BNP,and TNF-αwere significantly higher in the infected group than in the control patients,with statisti⁃cally significant differences(t=23.453,3.233,19.654,P<0.05).The area under the curve(AUC)of PCT,BNP,and TNF-αcom⁃bined to diagnose stroke with pulmonary infection was the largest,and the sensitivity and specificity could reach 90.0%and 85.3%.PCT,BNP,and TNF-αlevels were positively correlated with NIHSS score,and the difference was statistically significant(r=0.303,0.361,0.154,P<0.05).Conclusion:Serum PCT,BNP,and TNF-αlevels are significantly elevated in patients with stroke with pulmonary infection,reflecting neurological deficits,and can be diagnosed at an early stage of stroke with pulmonary infection.
