摘要
目的:探讨关节镜清理术与开放楔形胫骨高位截骨术(OWHTO)联合治疗内侧间室膝骨关节炎(KOA)的临床价值。方法:回顾性分析2019年2月─2022年2月东部战区海军医院82例内侧间室KOA患者临床资料,根据患者手术方案差异分为对照组与研究组,各41例。对照组接受单纯OWHTO治疗,研究组接受OWHTO联合关节镜清理术治疗。比较两组围手术期指标、美国特种外科医院膝关节(HSS)评分、Fugl-Meyer平衡量表(FMA)评分、美国西安大略与麦克马斯特大学骨关节炎指数(WOMAC)评分、临床疗效及并发症。结果:术前,两组HSS、FMA、WOMAC比较差异无统计学意义(P>0.05);术后3个月,研究组的HSS、FMA高于对照组,WOMAC低于对照组,差异均有统计学意义(P<0.05)。研究组的手术时间长于对照组,疗效优良率高于对照组(92.68%vs 73.17%),住院时间短于对照组,并发症发生率低于对照组(4.88%vs 24.39%),差异均有统计学意义(P<0.05)。结论:OWHTO联合关节镜清理术疗效良好,能够有效改善内侧间室KOA患者膝关节功能与运动能力,降低术后步态不稳发生风险,缩短患者术后康复时间。
Objective:To explore the clinical value of arthroscopic debridement combined with open wedge high tibial osteotomy(OWHTO)in the treatment of knee osteoarthritis(KOA)with medial compartment.Method:The clinical data of 82 patients KOA with medial compartment in the Naval Hospital of the Eastern Theater Command from February 2019 to February 2022 were retrospectively analyzed.According to the differences in surgical procedures,the patients were divided into control group and study group,with 41 patients in each group.The control group was treated with OWHTO alone,while the study group was treated with OWHTO combined with arthroscopic debridement.Perioperative indexes,hospital for special surgery knee(HSS)score,Fugl-Meyer balance assessment scale(FMA)score,Western Ontario and McMaster university(WOMAC)score,clinical efficacy and complications of the two groups were compared.Result:Before surgery,there were no significant differences in HSS,FMA and WOMAC between the two groups(P>0.05);3 months after surgery,the HSS and FMA of the study group were higher than those of the control group,and the WOMAC was lower than that of the control group,the differences were statistically significant(P<0.05).The surgery time of the study group was longer than that of the control group,the excellent and good rate of curative effect was higher than that of the control group(92.68%vs 73.17%),the hospital stay was shorter than that of the control group,and the incidence of complications was lower than that of the control group(4.88%vs 24.39%),the differences were statistically significant(P<0.05).Conclusion:The operation of OWHTO combined with arthroscopic debridement is effective.It can effectively improve the knee joint function and movement ability of patients with medial compartment KOA,reduce the risk of postoperative gait instability,and shorten the postoperative rehabilitation time of patients.
