摘要
目的采用Meta分析的方法系统评价我国患癌女性生育忧虑行为现状。方法检索PubMed、CINAHL、Embase、中国知网、维普数据库、万方数据库、中国生物医学文献数据库关于我国患癌女性生育忧虑的文献,检索时限为建库至2022年3月2日。由2名研究者按照标准独立进行文献筛选、资料提取和质量评价,采用Stata14.0和Graphpad Prism 8.0软件分别进行统计与绘图。结果共纳入22篇文献,包括5525例研究对象。Meta分析结果显示,我国患癌女性生育忧虑行为总分SMD=59.35,95%CI(58.04,60.66)。生育忧虑行为各维度评分:个人健康得分>子女健康得分>怀孕能力得分>不孕接受度得分>配偶知情得分>备孕能力得分。在我国患癌女性中,对罹患的癌症类型进行亚组分析,甲状腺癌患者生育忧虑总分>乳腺癌患者生育忧虑总分>淋巴瘤患者生育忧虑总分。对年龄进行亚组分析,小于40岁的患癌女性生育忧虑总分高于50岁的患癌女性生育忧虑总分。对地区进行亚组分析,生育忧虑行为总分华中地区>西北地区>华东地区>华北地区。处于化疗期间的患癌女性生育忧虑总分高于一般常模水平。结论我国患癌女性生育忧虑行为处于一个较高的水平,各地区生育忧虑水平不一,且其罹患的癌症类型、年龄与化疗影响其生育忧虑水平。
Objective To systematically evaluate the reproductive concerns behavior of Chinese women with cancer by means of meta-analysis.Methods PubMed,CINAHL,Embase,CNKI,VIP database,Wanfang Database and China Biomedical Literature Database were searched for literatures about reproductive concerns of Chinese women with cancer.The retrieval period was from the establishment of the database to March 2,2022.Two researchers independently conducted literature screening,data extraction and quality evaluation according to the standards.Stata 14.0 and Graphpad Prism 8.0 were used for statistics and mapping respectively.Results A total of 22 articles involving 5525 subjects were included.The results of meta-analysis showed that the total score SMD of reproductive concerns behavior among women with cancer in China was 59.35,95%CI(58.04,60.66).The scores of each dimension of reproductive concerns behavior:Personal health score>child health score>pregnancy ability score>infertility acceptance score>spouse informed score>pregnancy score.Among Chinese women with cancer,subgroup analysis of cancer types showed that the total score of reproductive concerns in thyroid cancer patients was higher than that in breast cancer patients,and the total score of reproductive concerns in lymphoma patients.Subgroup analysis of age showed that the total score of reproductive concerns among women with cancer aged less than 40years was higher than that among women with cancer aged 50 years.Subgroup analysis was conducted among regions.The total score of reproductive concerns behavior was Central China>Northwest China>East China>North China.The total score of reproductive concerns in women undergoing chemotherapy was higher than the normal level.Conclusion The reproductive concerns behavior of Chinese women with cancer is at a high level.The level of reproductive concerns varies from region to region,and the type of cancer,age and chemotherapy affect the level of reproductive concerns.
