肩袖撕裂及治疗后的肩胛骨运动学特征研究进展 被引量:4

摘要 肩袖撕裂(rotator cuff tear,RCT)通常会导致患者的肩部活动受限与疼痛,肩胛骨作为上肢的重要组成部分,现阶段的研究普遍认为RCT与肩胛骨动力障碍(scapular dyskinesis,SD)存在关联。通过在PubMed、Web of Science和CNKI数据库搜索,获取并整理关于RCT及治疗后肩胛骨运动的国内外相关文献。研究发现:1)当RCT存在时,会出现SD,表现为肩部外展时肩胛骨运动倾向于上旋增大与后倾减小,并且这一情况会随着患者临床症状的增强或肩部其他并发症状的存在而加剧;2)对RCT患者进行治疗后,随着主观症状的缓解,患者的肩胛骨运动也会恢复正常。但对于RCT尺寸较大的患者而言,相较于保守治疗,手术治疗后的肩胛骨运动障碍改善效果更为显著。对于RCT下肩胛骨运动学改变这一现象,主要考虑为肩胛骨代偿运动的结果。 Rotator cuff tears(RCT)usually lead to limited shoulder motion and pain.The scapular is an important part of the upper limbs,and studies at this stage generally agree that RCT are associated with scapular dyskinesis(SD).Through searching in PubMed,Web of Science,and CNKI databases,the relevant domestic and foreign literature on scapular motion after RCT and treatment were obtained and analyzed.It suggests that:1)When RCT exist,there is a SD,as evidenced by a tendency for increased upward rotation and decreased posterior tilt of the scapular during shoulder abduction,and this is exacerbated with the deterioration of clinical symptoms or the existence of other shoulder complications;2)after treatment of patients with RCT,the patient's scapular motion returns to normal as the subjective symptoms are relieved.However,for patients with large RCT,the improvement in scapular dyskinesia after surgical treatment is more significant compared to conservative treatment.For the phenomenon of scapular kinematic changes in RCT,the main consideration is the result of the compensatory movement of the scapular.
