情景模拟联合PBL教学在急诊重症监护室住院医师心肺复苏培训中的应用 被引量:14

Application of scenario simulation combined with PBL in cardio-pulmonary resuscitation teaching of EICU residents

摘要 目的 探讨情景模拟联合以问题为基础的学习(problem-based learning,PBL)在急诊重症监护室(emergency intensive care unit,EICU)住院医师心肺复苏(cardiopulmonary resuscitation,CPR)培训中的实际应用效果。方法 选取2021年1月至2021年10月在中山大学孙逸仙纪念医院EICU轮科的住院医师50名,随机分为两组,情景模拟联合PBL教学组(观察组)和传统教学组(对照组),每组25名。比较两组住院医师出科时理论考试及CPR成绩,并及时对实践技能、情景模拟联合PBL教学模式的认同情况进行问卷调查分析。结果 与对照组相比,观察组理论考试成绩无显著性差异,但CPR成绩及对EICU急诊教学工作满意度明显高于对照组(P<0.05)。问卷调查显示,观察组92%(23名)的住院医师认可情景模拟联合PBL教学,96%(24名)的住院医师认为能提高临床操作能力,92%(23名)的住院医师认为能提高临床思维能力,92%(23名)的住院医师认为能提高医患沟通和表达能力,96%(24名)的住院医师认为能提高团队合作能力,100%(25名)的住院医师意向再次使用。结论 情景模拟联合PBL教学能明显提高EICU住院医师CPR的培训效果。 Objective To explore the application effect of scenario simulation teaching combined with problem-based learning(PBL)in cardiopulmonary resuscitation(CPR)teaching for in EICU residents.Methods A total of 50 trained residents in EICU of Sun Yat-sen Memorial Hospital of Sun Yat-sen University from Janaury 2021 to October 2021 were selected and randomly divided into two groups:PBL teaching group(observation group)and traditional teaching group(control group),with 25 students in each group.The observation group and control group were further divided into 5 groups,namely 5 doctors as a group.The scores of theoretical test and CPR were compared between two groups,and the recognition of practical skills and scenario simulation combined with PBL teaching mode was analyzed by questionnaire before rotating out of the EICU Department.Results There was no significantly difference in theoretical test scores between two groups(P>0.05),but the CPR scores and the satisfaction of EICU emergency teaching of the observation group were significantly higher than throse of the control group (P< 0 . 05 ).The questionnaire survey showed that in the observation group, 92 % (n= 23 ) of the residents recognized scenariosimulation combined with PBL teaching, 96 % ( n= 24 ) of the residents believed that they could improve clinicaloperation ability, 92 % (n=23 ) of the residents believed that they could improve clinical thinking ability, 92 % (n= 23) of the residents believed that they could improve doctor-patient communication and ability of expression,96 % (n= 24 ) of the residents believed that they could improve team-work ability, and 100 % ( n= 100 ) of theresidents intended to use this way again. Conclusion Scenario simulation combined with PBL teaching modecan significantly improve the actual teaching effect of CPR in EICU training doctors.
