摘要
目的:观察沙利度胺综合干预治疗对输血依赖型β地中海贫血患者生长发育的临床效果。方法:选取2019年1月-2020年12月梧州市工人医院确诊并治疗的青少年输血依赖型β地中海贫血患者41例作为观察组,选取同期健康青少年41例作为对照组。观察组给予沙利度胺综合干预治疗,对照组未给予任何药物等干预治疗。比较两组研究对象一般资料、性激素水平、甲状腺激素水平,观察组是否出现不良反应。结果:研究前,观察组身高、体重、血红蛋白(Hb)水平均低于对照组,差异有统计学意义(P<0.05);研究后,观察组身高、体重、Hb水平均升高,差异有统计学意义(P<0.05),与对照组比较,差异无统计学意义(P>0.05);研究前,观察组患者促黄体生成素(LH)、促卵泡生成素(FSH)、雌二醇(E2)、睾酮(T)、孕酮(PROG)、泌乳素(PRL)水平低于对照组,差异有统计学意义(P<0.001);研究后,观察组患者LH、FHS、E2、T、PROG、PRL水平均升高,差异有统计学意义(P<0.05),与同期对照组比较,差异无统计学意义(P>0.05);研究前,观察组患者促甲状腺素(TSH)、游离三碘甲状腺素(FT3)、游离甲状腺素(FT4)水平低于对照组,差异有统计学意义(P<0.001);研究后,观察组患者TSH、FT3、FT4水平均提高,差异有统计学意义(P<0.05),与同期对照组比较,差异无统计学意义(P>0.05)。结论:沙利度胺综合干预治疗输血依赖型β地中海贫血可改善青少年患者生长发育现状,改善患者性激素水平及甲状腺激素水平。
Objective:To observe the clinical effect of thalidomide comprehensive intervention on the growth and development of patients with transfusion-dependentβthalassemia.Methods:A total of 41 adolescent patients with transfusion-dependentβthalassemia diagnosed and treated in Wuzhou Gongren Hospital from January 2019 to December 2020 were selected as the observation group,and 41 healthy adolescents during the same period were selected as the control group.The observation group was given thalidomide comprehensive intervention,and the control group received no drug and other intervention treatment.The general data,sex hormone levels and thyroid hormone levels of the two groups were compared,and whether adverse reactions occurred in the observation group.Results:Before study,the height,weight and hemoglobin(Hb)levels in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).After study,the height,weight and Hb levels in the observation group were increased and the difference was statistically significant(P<0.05),but the difference was not statistically significant compared with the control group(P>0.05).Before study,the levels of luteinizing hormone(LH),follicle-stimulating hormone(FSH),estradiol(E2),testosterone(T),progesterone(PROG),and prolactin(PRL)in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.001).After study,the levels of LH,FHS,E2,T,PROG and PRL in the observation group were increased and the difference was statistically significant(P<0.05),but the difference was not statistically significant compared with the control group during the same period(P>0.05).Before study,the levels of thyrotropin(TSH),free triiodothyroxine(FT3)and free thyroxine(FT4)in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.001).After study,the levels of TSH,FT3 and FT4 in the observation group were increased and the difference was statistically significant(P<0.05),but the difference was not statistically significant compared with the control group during the same period(P>0.05).Conclusion:The comprehensive intervention of thalidomide in the treatment of transfusion-dependentβthalassemia can improve the growth and development status of adolescent patients,and improve the level of sex hormones and thyroid hormones in patients.
