摘要
乳痈病位在乳房,病因病机多为肝之疏泄失畅,乳汁分泌或排出失调;或因饮食不节,胃中积热,肝胃失和,郁热阻滞乳络;或因乳头外伤,乳络不畅,或断乳不当,引起乳汁瘀滞不得出;或外邪侵袭,闭阻乳络所致。乳痈与足阳明胃经、足厥阴肝经、足少阳胆经密不可分,治疗原则为疏肝解郁,清热和胃,尽快将积乳排出。肩井穴首载于《针灸甲乙经》,属足少阳胆经的经穴之一,可调畅气机、疏泄肝胆,气血上行以滋养乳房,乳络通畅,清热和胃,除胃中积热,消肿散结止痛,标本兼治,切中乳痈治疗之关键,穴简效宏,为治疗乳痈要穴。肩井穴治疗乳痈的机制,从中医的角度解析为调畅气机、清热平肝和胃,气血上行滋养乳房,促使乳汁分泌;从现代医学角度看,其机制可能为调整产妇的异常体态、松解乳房及周围组织压力,通过神经内分泌反射调节乳汁分泌,并通过免疫机制发挥镇痛作用。
Breast carbuncle is located in the breast,and its etiology and pathogenesis are often due to poor liver function,as well as dysregulation of milk secretion or excretion;Or due to improper diet,accumulation of heat in the stomach,disharmony between the liver and stomach,stagnation of heat blocking the milk collaterals;Or due to nipple trauma,obstructed milk collaterals,or improper weaning,resulting in milk failing to spill out;Or invasion of external pathogens,resulting in blockage of the milk collaterals.Breast carbuncle is closely related to the stomach meridian of foot yang brightness,the liver meridian of foot reverted yin,and the gallbladder meridian of foot lesser yang.The treatment principle is to relieve liver and remove stagnation,clear heat and harmonize the stomach,and quickly expel accumulated milk.Jianjing(GB 21)point was first recorded in The ABC Classic of Acupuncture and Moxibustion,which is one of the points of the gallbladder meridian of foot lesser yang.It can regulate the qi mechanism,relieve the liver and gallbladder,nourish the breast by ascending the qi and blood,dredge the milk collaterals,clear heat and harmonize the stomach,remove the accumulated heat in the stomach,diminish swelling,disperse knots and relieve pain,treat both symptoms and root causes,and hit the key point for the treatment of breast carbuncle.The point is simple and effective,which is an important treating breast carbuncle.The mechanism of treating breast carbuncle at Jianjing point,from the perspective of TCM,is that it can regulate qi movement,clear heat,pacify the liver,harmonize the stomach,and nourish the breast with ascending qi and blood,promoting milk secretion.From the perspective of modern medicine,its mechanism may be to adjust the abnormal posture of postpartum women,relieve pressure on the breasts and surrounding tissues,regulate milk secretion through neuroendocrine reflexes,and exert analgesic effects through immune mechanisms.
