摘要
目的 探讨超声血流参数对浸润性/非浸润性乳腺癌的鉴别价值。方法 回顾性分析鹤壁市人民医院于2019年1月至2022年8月收治的经病理学检查确诊的60例乳腺癌患者,根据WHO组织学病理分类将其分为浸润性乳腺癌(浸润组,42例)和非浸润性乳腺癌(非浸润组,18例)。所有患者均于入院后进行超声检查,比较两组患者的超声血流参数[血流搏动指数(PI)、血流阻力指数(RI)、峰值流速(V_(max))],采用受试者工作特征(ROC)曲线分析PI、RI、V_(max)以及联合对浸润性乳腺癌与非浸润性乳腺癌的鉴别诊断效能。结果 浸润组、非浸润组患者年龄、体重指数、肿瘤位置、疾病分期、分化程度、淋巴结转移情况、病灶直径比较差异无统计学意义(P>0.05);浸润组PI、RI低于非浸润组(P<0.05),V_(max)高于非浸润组(P<0.05);ROC结果显示,PI、RI、V_(max)三者联合鉴别浸润性乳腺癌与非浸润性乳腺癌的灵敏度、特异度、曲线下面积(AUC)分别为95.24%、83.33%、0.954,其中联合鉴别诊断的灵敏度和AUC均高于单独诊断(P<0.05),特异度与单独诊断比较差异均无统计学意义(P>0.05)。结论 浸润性乳腺癌的PI、RI水平相对较低,V_(max)水平相对较高,PI、RI、V_(max)对浸润性乳腺癌与非浸润性乳腺癌均具有一定的鉴别诊断价值,但三者联合鉴别诊断价值更高。
Objective To explore the value of ultrasound blood flow parameters in differential diagnosis of invasive/non-invasive breast cancer.Methods Sixty patients with breast cancer diagnosed by pathology admitted to Hebi People’s Hospital from January 2019 to August 2022 were retrospectively analyzed.According to the WHO histological and pathological classification,they were divided into invasive breast cancer(invasive group,42 cases)and non-invasive breast cancer(non-invasive group,18 cases).All patients underwent ultrasound examination after admission.The ultrasonic blood flow parameters[blood flow pulsation index(PI),blood flow resistance index(RI),maximum velocity(V_(max)]of the two groups were compared.The receiver operating characteristic(ROC)curve was used to analyze the differential diagnostic efficacy of PI,RI,V max and combination in invasive breast cancer and non-invasive breast cancer.Results There was no statistically significant differences in age,BMI,tumor location,disease stage,degree of differentiation,lymph node metastasis and lesion diameter between the invasive group and the non-invasive group(P>0.05).PI and RI in the invasive group were lower than those in the non-invasive group(P<0.05),and V_(max) was higher than that in the non-invasive group(P<0.05).ROC results showed that the sensitivity,specificity and area under the curve(AUC)of PI,RI and V_(max) in combined differentiating invasive breast cancer from non-invasive breast cancer were 95.24%,83.33%and 0.954,respectively,and the sensitivity and AUC of the combined differential diagnosis were higher than those of single diagnosis(P<0.05),but there was no statistical significance between the specificity and single diagnosis(P>0.05).Conclusion The levels of PI and RI in invasive breast cancer are relatively low,but the level of V_(max) is relatively high.PI,RI and V_(max) all have a certain value in differential diagnosis between invasive breast cancer and non-invasive breast cancer,but their combined diagnostic value is higher.
