后交叉韧带保留型全膝关节置换术对患者满意度及正常行走身体稳定性的影响

Effects of Posterior Cruciate Ligament-sparing Total Knee Arthroplasty on Patient Satisfaction and Normal Walking Body Stability

摘要 目的:探讨后交叉韧带保留型全膝关节置换术(CR-TKA)对患者满意度及正常行走的身体稳定性影响。方法:随机选取18例膝骨关节炎患者,利用主观评分量表、Kwon 3D三维动作解析系统及Kistler三维测力台分别记录CR-TKA前后的OKS、WOMAC评分、正常行走的步态参数及身体稳定性参数,对CR-TKA的效果进行主、客观的量化评定。结果:与术前相比,CR-TKA后的OKS评分、WOMAC评分、压心前后、内外侧位移均显著下降(P<0.05),步长、步速、膝关节活动度均显著增加(P<0.05),步宽、质心前后、内外侧位移未见显著性差异(P>0.05)。结论:CR-TKA后患者满意度较高;CR-TKA有效增强患者正常行走的步态表现能力,改善关节活动表现,提高正常行走的身体稳定性。 Objective:To investigate the effect of total knee arthroplasty with posterior cruciate ligament preservation(CR-TKA)on patient satisfaction and physical stability of normal walking.Methods:18 patients with knee osteoarthritis were randomly selected.The subjective rating scale,Kwon 3D three-dimensional motion analysis system and Kistler three-dimensional force platform were used to record the OKS,WOMAC scores,gait parameters and body stability parameters before and after CR-TKA,and the effect of CR-TKA was evaluated subjectively and objectively.Results:Compared with before operation,the OKS score,WOMAC score,anterior-posterior and medial-lateral displacements of the pressure center after CR-TKA were significantly decreased(P<0.05),and the step length,walking speed and knee joint mobility were significantly increased(P<0.05).There was no significant difference in step width,centroid anterior-posterior and medial-lateral displacements(P>0.05).Conclusion:The satisfaction of patients after CR-TKA is higher;CR-TKA can effectively enhance the gait performance of patients with normal walking,improve joint activity performance,and improve the physical stability of normal walking.
