摘要
目的:探讨地佐辛联合异丙酚在宫腔镜检查中的应用效果及其对氧化应激和呼吸抑制的影响。方法:选取2018年1月—2021年1月南京市红十字医院收治的进行宫腔镜检查的患者82例为观察对象,根据随机数字表法分为观察组与对照组,各41例。对照组采用常规麻醉,观察组采用异丙酚联合地佐辛。比较两组患者氧化应激指标、麻醉效果、血流动力学指标、不良反应发生率。结果:检查后1 d,两组患者丙二醛(MDA)、过氧化氢(H2O2)水平均升高,总抗氧化态(TAS)水平均降低,差异有统计学意义(P<0.05);观察组患者H2O2、MDA水平低于对照组,TAS水平高于对照组,差异有统计学意义(P<0.05);观察组患者视觉疼痛模拟评分低于对照组,麻醉起效时间、意识恢复时间、呼之睁眼时间短于对照组,异丙酚用量低于对照组,差异有统计学意义(P<0.05);扩宫时,两组患者心率(HR)及平均动脉压水平均降低,差异有统计学意义(P<0.05);观察组HR水平高于对照组,差异有统计学意义(P<0.05);观察组患者不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:使用异丙酚复合地佐辛在宫腔镜检查中的麻醉效果较好,不良反应少,氧化应激指标及血流动力学指标较优。
Objective:To explore the application effect of dezocine combined with propofol in hysteroscopy and its effect on oxidative stress and respiratory depression.Methods:From January 2018 to January 2021,eighty-two patients who underwent hysteroscopy in Nanjing Red Cross Hospital were selected as observation objects.According to the random number table method,they were divided into observation group and control group,with 41 cases in each group.The control group received conventional anesthesia,and the observation group received propofol combined with dezocine.The oxidative stress indicators,anesthesia effect,hemodynamic indicators,and incidence of adverse reactions were compared between the two groups.Results:One day after the examination,the levels of malondialdehyde(MDA)and hydrogen peroxide(H2O2)in the two groups increased,while the levels of total antioxidant status(TAS)decreased,and the difference was statistically significant(P<0.05).The levels of H2O2 and MDA in the observation group were lower than those in the control group,and the levels of TAS were higher than those in the control group,the difference was statistically significant(P<0.05).The visual pain simulation score of the observation group was lower than that of the control group,the anesthesia onset time,consciousness recovery time,and eye-opening time were shorter than those of the control group,and the dosage of propofol was lower than that of the control group,the difference was statistically significant(P<0.05).During uterine dilatation,the heart rate(HR)and mean arterial pressure of the two groups decreased,and the difference was statistically significant(P<0.05).The HR level of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Propofol combined with dezocine has better anesthesia effect in hysteroscopy,fewer adverse reactions,better oxidative stress indicators and hemodynamic indicators.
