摘要
目的 分析慢阻肺患者临床接受糖皮质激素联合多索茶碱治疗期间采取集束化护理取得的临床效果。方法 在本院2019年1月-2021年3月内接受糖皮质激素联合多索茶碱治疗慢阻肺患者中随机抽选76例,随机对其进行分组,对照组38例接受常规临床护理,试验组38例接受集束化护理,比较两组患者肺功能指标、CAT评分、康复效果、用药依从性及护理满意度。结果 试验组患者FEV1、FVC、FEV1/FVC三项肺功能指标明显高于对照组患者,组间差异显著,具有统计学意义(P<0.05);试验组患者在院治疗时间明显更短,对照组相对较长,两组比较差异显著,统计学意义明显(P<0.05);在院期间两组患者接受护理后服药依从性并不存在显著性差异,无统计学意义(P>0.05)。出院之后试验组患者服药依从性评分明显比对照组高,两组存在显著性差异(P<0.05);试验组患者出院时、出院3个月后、出院6个月后生活质量明显优于对照组患者,两组比较具有显著性统计学差异(P<0.05);试验组患者疾病复发率明显低于对照组患者,肺功能优良率明显优于对照组患者,两组比较差异显著,具有统计学意义(P<0.05);试验组患者护理总满意度明显高于对照组患者,组间数据比较差异显著,具有统计学意义(P<0.05)。结论 集束化护理在慢阻肺患者接受糖皮质激素联合多索茶碱治疗期间应用取得了比较理想的效果,护理后患者肺功能指标得到改善,生活质量得到提升,用药依从性得到提高,应用价值明显。
Objective To analyze clinical effect of cluster nursing for chronic obstructive pulmonary disease(COPD)patients with treated by glucocorticoids combined with doxofylline.Methods The paper chose 76 patients with chronic obstructive pulmonary disease(COPD)from January 2019 to March 2021,and divided them into two groups randomly.Control group was treated with routine clinical nursing,while experimental group with cluster nursing.Lung function indicators,CAT scores,rehabilitation effect,medication compliance,and nursing satisfaction between two groups were compared.Results FEV1,FVC,and FEV1/FVC lung function indicators in experimental group were significantly higher than control group,with significant difference between the groups and statistical significance(P<0.05).Treatment time in experimental group was significantly shorter,while control group was relatively longer.Difference between two groups was significant and statistically significant(P<0.05).There was no significant difference in medication adherence between two groups receiving nursing during the hospital period,with no statistically significant difference(P>0.05).After discharge,medication compliance scores of experimental group were significantly higher than control group,with significant difference between two groups(P<0.05).Life quality of experimental group was significantly better than control group at discharge,3 months after discharge,and 6 months after discharge,with significant statistical difference between two groups(P<0.05).Disease recurrence rate of experimental group was significantly lower than control group patients,excellent and good lung function rate was significantly higher than control group patients,difference between two groups was significant and statistically significant(P<0.05).Overall nursing satisfaction degree of experimental group was significantly higher than control group patients,there was significant difference in data comparison between the groups,with statistical significance(P<0.05).Conclusion Cluster nursing can achieve ideal results for chronic obstructive pulmonary disease(COPD)patients treated by glucocorticoids combined with doxofylline.After nursing,lung function indicators and life quality,medication compliance has been improved,with significant application value.
