摘要
目的 调查慢性气道疾病患者稳定期干粉吸入装置吸入技术掌握情况,并分析其影响因素。方法 选取2020年1月至2020年12月在常熟市第一人民医院呼吸与危重症医学科门诊就诊的慢性阻塞性肺疾病或哮喘患者179例,通过问卷评分的方法,对患者吸入技术掌握情况进行评估,分析4种吸入装置的常见错误步骤,采用二元logistic回归分析吸入技术情况的影响因素。结果 55例(30.7%)患者能完全正确掌握吸入技术,操作错误率前3位的步骤依次为:远离吸嘴,尽可能充分呼气(44.9%);将装置从嘴边移开,屏气5~10 s后缓慢呼气(33.3%);用完吸入剂后及时漱口(29.5%)。二元logistic回归分析显示,病程、依从性情况是慢性气道疾病患者吸入技术错误的影响因素(P<0.05)。结论 慢性气道疾病患者干粉吸入装置吸入技术正确率较低,医务人员应规范吸入技术的随访路径,强化患者的吸入技术,以提高吸入装置操作的正确性。
Objective To investigate the mastery of inhalation technology of dry powder inhalation device for patients with chronic airway disease in stable period,and analyze its influential factors.Methods From January to December 2020,a total of 179 patients with chronic obstructive pulmonary disease or asthma in the outpatient department of Pulmonary and Critical Care Medicine in Changshu NO.1 People's Hospital were recruited for the study.Through the method of questionnaire scoring,the patient's mastery of inhalation technology was evaluated,analyze common wrong steps of 4 inhalation devices,using binary logistic regression to analyze the influential factors of inhalation technology.Results 55 patients(30.7%)can correctly master the inhalation technique.Operation steps for ranking the top 3 error rates were as follows:stay away from the mouthpiece and exhale as fully as possible(44.9%);remove the device from the mouth,hold your breath for 5-10 s and exhale slowly(33.3%);after using the inhaler,gargle in time(29.5%).Binary logistic analysis showed that the course of disease and compliance are the influential factors of inhalation technique errors in patients with chronic airway diseases(P<0.05).Conclusion The correct rate of inhalation technology of dry powder inhalation device for patients with chronic airway disease is low.Medical personnel should standardize the follow-up path and strengthen the inhalation technology of patients to improve the accuracy of the operation of inhalation device.
