摘要
目的观察槐耳颗粒治疗肉芽肿性小叶性乳腺炎肿块期的有效性并初步探讨其机制。方法选取2022年1月至2022年9月在浙江省中医院乳腺门诊就诊符合肿块期阳虚痰凝证的肉芽肿性小叶性乳腺炎患者40例,采用随机数字表法分为对照组(n=20)和试验组(n=20),对照组采用阳和汤加减治疗,试验组在对照组的基础上加用槐耳颗粒。观察两组患者治疗前后的临床症状体征积分、疗效、血清炎症及免疫指标水平变化情况。结果治疗前,两组患者一般资料、临床症状体征积分、血清各项炎症及免疫指标比较,差异无统计学意义(P>0.05)。治疗后两组相比,试验组总有效率更高,且C反应蛋白、白细胞介素(interleukin,IL)-2、IL-6、CD4^(+)/CD8^(+)降低程度更大,差异均有统计学意义(P<0.05)。两组治疗后的白细胞、IL-4、免疫球蛋白(immunoglobulin,Ig)A、IgG、IgM及补体C3、C4虽有差异,但差异无统计学意义(P>0.05)。结论槐耳颗粒治疗肉芽肿性小叶性乳腺炎有一定的疗效,或许跟其能改善患者自身免疫力,缓解炎症反应有关。
Objective To observe the effectiveness of Huai’er granules in the treatment of granulomatous lobular mastitis in the mass stage and preliminarily explore its mechanism.Methods A total of 40 patients with granulomatous lobular mastitis who met the inclusion and exclusion criteria in mammary gland of Zhejiang Provincial Hospital of Chinese Medicine from January 2022 to September 2022 were selected and divided into the control group(n=20)and the test group(n=20)according to the random number table method,the control group were treated with Yanghe decoction,and the test group were additionally treated with Huai’er granule.The scores of clinical symptoms and signs,efficacy,serum inflammatory and immune indexes were observed before and after treatment in two groups.Results Before treatment,there were no statistical differences between the two groups in terms of general information,clinical symptoms and signs scores,and various serum inflammatory and immunological indicators(P>0.05).After treatment,the test group had a higher total effective rate and lower levels of C reactive protein,interleukin(IL)-2,IL-6,and CD4^(+)/CD8^(+),the differences were statistically significant(P<0.05).There were no significant differences in white blood cell,IL-4,immunoglobulin(Ig)A,IgG,IgM,complement 3 and complement 4 levels between the two groups after treatment(P>0.05).Conclusion Huai’er granules have certain curative effect on granulomatous lobular mastitis,which may be related to improving the patients'own immunity and alleviating inflammation.
