摘要
目的 观察植入125I粒子后患者对周围环境的辐射影响。方法 纳入62例接受125I粒子植入术治疗的肿瘤患者,分别于有护具防护及无防护状态下测量并比较距离患者0 m(术侧及对侧手腕处)、1 m及2 m处吸收剂量的差异,并与核医学放射防护要求规定的公众(2.5μSv/h)、放射工作人员剂量限值(25μSv/h)及术后手术室本底剂量进行对比。结果 距离相同时,防护状态下患者周围环境吸收剂量明显低于无防护状态(P均<0.001)。无防护状态下,随距离增加,吸收剂量明显减小(P<0.01);距离0 m处术侧及对侧吸收剂量均高于公众剂量限值及本底(P均<0.001)而低于放射工作人员剂量限值(P均<0.001),距离1 m及2 m处吸收剂量均低于公众剂量限值(P均<0.01)而高于本底(P均<0.001)。防护状态下不同距离处吸收剂量均低于公众剂量限值(P均<0.001)且与本底剂量差异无统计学意义(P均>0.05)。结论 植入125I粒子后的辐射效应随距离增加而显著衰减;在手术区域对侧操作可有效降低所受辐射,距离患者2 m处吸收剂量明显低于公众剂量限值而接近本底剂量。
Objective To observe the impact of patients after 125 I seeds brachytherapy of radiation on surrounding environment.Methods A total of 62 tumor patients who underwent 125 I seeds brachytherapy were enrolled.The absorbed doses at 0 m(operative and contralateral wrist),1 m and 2 m from the patient were measured and compared with the public dose(2.5μSv/h),dose limit for radiation workers(25μSv/h)and the natural background radiation under both protected and unprotected settings,respectively.Results The absorbed doses measured under protected condition were significantly lower than those without protection at the same distances(all P<0.001).Without protection,the absorbed dose decreased significantly with the increase of distance(P<0.01),at 0 m on both the operative and contralateral side were higher than the public dose limit and the background(both P<0.001)but lower than the dose limit for radiation workers(both P<0.001),while at 1 m and 2 m were both lower than the public dose limits(both P<0.01)but still higher than the background dose(both P<0.001).Under protection,the absorbed dose at different distances were all lower than the public dose limit(all P<0.001),but no significant difference was found between the absorbed dose and the background dose(all P>0.05).Conclusion After brachytherapy,the radiation effect of 125 I decreased significantly with increase of distance.Operating on the opposite side of the operating area could effectively reduce radiation exposure.The absorbed dose at 2 m from patient was significantly lower than the public dose limit and closed to the background dose.
