摘要
《金匮要略》为东汉末年医圣张仲景所著的一本论述杂病的专书,其中“妇人三篇”开中医妇科辨证论治之先河,对妇人腹痛诸疾的论治颇为详实,分述了妊娠、产后及杂病腹痛。依腹痛病因病机、病位、病性不同,而治法多变;仲师审因论治、审证求因,结合病因辨证;据证论理、依理立法、按法立方,设方二十余首,配伍严谨、药简效宏、用药精当;辨治妇人腹痛有其独特之处,为临床医家所遵从和沿用至今。管窥仲师施治本病的遣方用药,溯其病性亦为虚实两端;虚则为冲任虚寒、血虚寒凝、脾胃虚寒、肾阳亏虚,分别施以温补冲任、养血散寒、建中培土、温补肾阳之法;实则多为瘀血内结、阳明里实、风血相搏、气滞血凝、肝郁血滞、血水互结,分别施以活血通经、通腹泄热、活血祛风、行气活血、疏肝通络、化瘀利水之法;虚实夹杂者多为肝郁脾虚、血虚湿热、脾虚寒湿、热利伤阴,以疏肝健脾、养血清热利湿、温中健脾除湿、清热养阴止利为治则。其病机亦为“不通则痛”和“不荣则痛”,仲景以“通”立法,以调畅气血为通,以补以温为通,以清热泻下为通,以调和脏腑为通。
Synopsis of the Golden Chamber is a special book on miscellaneous diseases written by medical sage ZHANG Zhongjing in the late Eastern Han Dynasty.Among them,three articles by women are the first of its kind in TCM gynecology syndrome differentiation and treatment,the treatment of women's abdominal pain and various diseases is quite detailed,abdominal pain in pregnancy,post partum and miscellaneous diseases described.Depending on the etiology,pathogenesis,disease location,and nature of abdominal pain,the treatment methods vary.Master ZHONG examines the cause,discusses the treatment,examines the evidence and seeks the cause,and combines the etiology and syndrome differentiation.Based on evidence,legislating,and formulating prescriptions,there are more than 20 prescriptions,which are rigorous in compatibility,simple and effective in medicine,and precise in medicine.Differentiation and treatment of abdominal pain in women has its own unique features,which are followed and used by clinicians to this day.I do not take the risk,observe Master ZHONG's prescriptions and medicines for treating the root of the disease,traced back to the nature of the disease are also the two ends of the virtual and the real.Deficiency is Chongren deficiency cold,blood deficiency and cold coagulation,spleen and stomach deficiency cold,kidney-Yang deficiency,the methods of warming and tonifying Chongren,nourishing blood and dispelling cold,building up soil and warming kidney-Yang are applied respectively.Evidences,most of them are blood stasis,internal Yangming,wind and blood,Qi stagnation and blood coagulation,liver stagnation and blood stagnation,and blood and water intertwined,respectively apply the methods of promoting blood circulation and removing heat,promoting blood circulation and expelling wind,activating Qi and promoting blood circulation,soothing liver and dredging collaterals,and removing blood stasis and diuresis.The syndromes of deficiency and excess are mostly liver stagnation and spleen deficiency,blood deficiency and damp-heat,spleen deficiency and cold dampness,and heat injuring the Yin,to soothe the liver and invigorate the spleen,nourish the blood serum and disperse dampness,warm the middle to invigorate the spleen and dehumidify,clear heat and nourish Yin and stop the benefit.Its pathogenesis is also“obstruction leads to pain”and“failure leadsto pain”.Zhongjing legislates“unblocking”,regulating Qi and blood,tonifying andwarming,clearing heat and purging,and harmonizing viscera.
