摘要
目的:研究骨折内固定术和全髋关节置换术2种手术方法用于女性老年股骨颈骨折治疗上的效果。方法:选取我院骨外二科2019年7月-2020年7月就诊的80例女性老年股骨颈骨折患者,用随机数字表法将其分为2组,各40例。对照组和观察组的手术方法分别是骨折内固定术、全髋关节置换术,对比2种治疗方式的临床效果。结果:通过对比2组患者的骨折愈合情况,观察组的骨折愈合优良率与对照组相比相对较高,2组之间的比较,有显著差异(P<0.05);与对照组相比,观察组的手术时间短、出血量少、术后下床活动时间短、住院时间短,并发症少,2组相比有明显差异;观察组的Harris评分和BI指数与对照组相比相对较高,VAS评分较对照组低,2组对比,差异显著(P<0.05)。结论:对于女性老年股骨颈骨折患者,临床上采用全髋关节置换术治疗获得的效果优于骨折内固定术,骨折愈合效果更佳,手术指标更优化,术后身体恢复更快,且并发症少,可见此种手术方法兼具有效性和安全性。因此,值得在临床上进一步推广和应用。
Objective:To study the effect of internal fracture fixation and total hip arthroplasty on the treatment of femoral neck fractures in elderly women.Methods:A total of 80 elderly female patients with femoral neck fracture who were treated in the Second Department of Orthopedics in our hospital from July 2019 to July 2020 were selected and divided into two groups by random number table method,with 40 cases in each group.The surgical methods of the control group and the observation group were fracture internal fixation and total hip arthroplasty,respectively,and the clinical effects of the two treatment methods were compared.Results:By comparing the fracture healing of the two groups of patients,the excellent and good rate of fracture healing in the observation group was relatively higher than that in the control group,and there was a significant difference between the two groups(P<0.05);Compared with the control group,the operation time of the group was shorter,the amount of blood loss was shorter,the time of getting out of bed was shorter,the hospitalization time was shorter,and the complications were less,and there were significant dfferences between the two groups;the Harris score and BI index of the observation group were relatively higher than those of the control group.,VAS score was lower than the control group,the difference between the two groups was significant(P<0.05).Conclusion:For female elderly patients with femoral neck fracture,the clinical effect of total hip arthroplasty is better than that of internal fixation,the fracture healing effect is better,the surgical indicators are more optimized,the postoperative recovery is faster,and the complications are less.It can be seen that this surgical method is both effective and safe.Therefore,it is worthy of further promotion and application in clinical practice.
