摘要
目的:研究在社区原发性高血压患者管理中应用医联体管理模式对其血压控制及健康行为的影响。方法:选取2021年1月至2022年1月期间深圳市大鹏新区3个社区200例原发性高血压患者,随机数原则分为对照组与观察组,各100例。对照组患者应用常规管理模式,观察组患者应用医联体管理模式,比较分析两组患者血压水平、高血压相关知识知晓情况及健康行为评分。结果:干预后与对照组患者血压水平(舒张压、收缩压)相比,观察组患者水平更低,差异具有统计学意义(P<0.05);干预后观察组在症状、危险因素知晓情况优于对照组,差异具有统计学意义(P<0.05);干预后两组患者吸烟行为评分比较,差异无统计学意义(P>0.05);相较对照组患者,观察组患者低盐、低脂饮食评分更高,差异具有统计学意义(P<0.05)。结论:在社区原发性高血压患者管理中,应用医联体管理模式效果理想,可促进患者血压水平改善,加强患者自我管理能力,利于健康行为的养成。
Objective To study the effect of Medical Union management model on blood pressure control and health behavior in the management of patients with essential hypertension in community.Methods 200 patients with essential hypertension in three communities in Dapeng new district,Shenzhen from January 2021 to January 2022 were randomly divided into a control group(100 cases)and an oberservation group(100 cases).The former applied routine management mode and the latter applied medical union management mode.The blood pressure level,awareness of hypertension related knowledge and health behavior scores of the two groups were compared and analyzed.Results After the intervention,the blood pressure level in the oberservation group was lower than that in the control group(diastolic and systolic blood pressure)with statisticed difference(P<0.05).After the intervention,compared with the control group,the awareness of symptoms and risk factors in the observation group was significantly better(P<0.05).After the intervention,there was no significant difference in smoking behavior scores between the two groups(P>0.05).Compared with the control group,the observation group had higher scores of low salt,low-fat diet with statisticed differences(P<0.05).Conclusion In the management of patients with essential hypertension in the community,the application of Medical Union management mode has ideal effect,which can promote the improvement of patients'blood pressure level,strengthen patients'self-management ability,facilitate the cultivation of healthy behavior,and has high feasibility of popularization.
