摘要 目的探讨基于情绪ABC理论下Teach-back健康教育在内镜逆行胰胆管造影(ERCP)+内镜十二指肠乳头括约肌切开术(EST)患者中的应用价值。方法2020年1~12月选取徐州医科大学附属医院ERCP+EST术患者92例,采用随机双盲法将患者分为观察组及对照组,各46例。对照组行常规健康指导,观察组实施情绪ABC理论下Teach-back健康教育,比较两组围术期自我效能、不良情绪、治疗依从性、术后并发症、预后情况及满意度。结果干预后观察组自我效能评分、治疗依从性评分、满意度评分较对照组明显提高,差异有统计学意义(P<0.05),干预后观察组汉密尔顿焦虑量表(HAMA)评分、汉密尔顿抑郁量表(HAMD)评分明显低于对照组,差异有统计学意义(P<0.05)。观察组术后肛门排气时间、首次下床时间、住院时间明显短于对照组,差异有统计学意义(P<0.05)。结论基于情绪ABC理论下Teach-back健康教育能有效减轻ERCP+EST患者围术期不良情绪,提高患者治疗依从性,降低患者术后并发症发生率,促进患者术后康复,提高患者治疗满意度。 Objective To explore the application value of Teach-back health education based on the emotional ABC theory in patients undergoing endoscopic retrograde cholangiopancreatography(ERCP)+endoscopic duodenal papillary sphincterotomy(EST).Methods A total of 92 cases of ERCP+EST patients were selected,and the patients were divided into observation group and control group by random double-blind method,with 46 cases in each group.The control group received regular health guidance,while the observation group implemented Teach-back health education based on the emotional ABC theory.The perioperative self-efficacy,bad mood,treatment compliance,postoperative complications,prognosis and satisfaction were compared between the two groups.Results After intervention,the self-efficacy score,treatment compliance score and satisfaction score of the observation group were significantly improved compared with the control group,and the differences were statistically significant(P<0.05).After intervention,Hamilton Anxiety Scale(HAMA)scores and Hamilton Depression Scale(HAMD)scores of the observation group were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).The postoperative anal exhaust time,the first time to get out of bed and the length of hospital stay in the observation group were significantly shorter than those in the control group,and the differences were statistically significant(P<0.05).Conclusions Teach-back health education based on the emotional ABC theory can effectively reduce the perioperative bad mood of ERCP+EST patients,improve patients'compliance with treatment,reduce the incidence of postoperative complications,promote postoperative recovery,and improve patients'satisfaction with treatment degree.
