摘要
目的探究苏黄止咳胶囊联合孟鲁司特钠对感冒后咳嗽患者中医证候、免疫功能的影响。方法选取90例感冒后咳嗽患者,采用随机数字表法分为2组。对照组45例予苏黄止咳胶囊治疗,治疗组45例予苏黄止咳胶囊联合孟鲁司特钠治疗,2组均连续治疗7天后统计疗效。比较2组治疗前后日间和夜间咳嗽症状评分、咳嗽视觉模拟评分(VAS)、呼出气一氧化氮(FeNO)及CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、CD8^(+)水平、第1秒用力呼气容积(FEV 1)占预计值百分比(FEV 1%pred)、用力肺活量(FVC)、FEV 1/FVC、中医证候评分,观察并记录2组不良反应发生率。结果治疗后,2组日间、夜间咳嗽症状评分及咳嗽VAS、FeNO水平均低于本组治疗前(P<0.05),且治疗组均低于对照组(P<0.05)。治疗后,2组CD8^(+)水平均低于本组治疗前(P<0.05),CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均明显高于本组治疗前(P<0.05),且2组组间比较差异也均有统计学意义(P<0.05)。治疗后,2组FVC、FEV 1%pred及FEV 1/FVC均较本组治疗前升高(P<0.05),且治疗组均高于对照组(P<0.05)。治疗后,2组中医证候咯痰、咳嗽、咽痒评分均低于本组治疗前(P<0.05),且治疗组均低于对照组(P<0.05)。治疗组总有效率为97.78%(44/45),不良反应发生率为4.44%(2/45),对照组分别为77.78%(35/45)、20.00%(9/45),治疗组总有效率高于对照组(P<0.05),不良反应发生率低于对照组(P<0.05)。结论采用苏黄止咳胶囊联合孟鲁司特钠联合治疗感冒后咳嗽,可有效改善患者免疫功能、肺功能,减轻咳嗽、咯痰等中医症状,降低FeNO水平及不良反应发生率,效果显著。
Objective To explore the effect of Su Huang cough capsule combined with montelukast sodium on traditional Chinese medicine(TCM)syndrome and immune function in patients with postinfectious cough.Methods Ninety patients with postinfectious cough were randomized 1∶1 to receive Su Huang cough capsule(the control group)or Su Huang cough capsule^(+)montelukast sodium(the treatment group).Treated for 7 days,the aim was to compare daytime and nighttime cough score,cough visual analog scale(VAS),fractional exhaled nitric oxide(FeNO),CD3^(+),CD4^(+),CD4^(+)/CD8^(+),CD8^(+),percent predicted forced expiratory volume in one second(FEV 1%pred),forced vital capacity(FVC),FEV 1/FVC,TCM syndrome scores,as well as adverse incidence.The curative effect was assessed.Results After treatment,the scores of daytime and nighttime cough symptom,VAS and FeNO in the both groups were lower than those before treatment(P<0.05),and those in the treatment group were more common(P<0.05).Decreased CD8^(+)and increased CD3^(+),CD4^(+),CD4^(+)/CD8^(+)were detected in the both groups(all P<0.05),with significant differences between groups(P<0.05).FVC,FEV 1%pred and FEV 1/FVC in groups were increased when compared with those before treatment(P<0.05),and which in the treatment group were more prevalent(P<0.05).TCM syndromes scores(expectoration,cough,pharyngeal itching)in groups were lower than those before treatment(P<0.05),and which were lower in the treatment group than in the control group(P<0.05).The total effective rate in the treatment group was higher than that in the control group(97.78%[44/45]vs 77.78%[35/45],P<0.05).The adverse incidence in the treatment group was lower than that in the control group(4.44%[2/45]vs 20.00%[9/45],P<0.05).Conclusion For patients with postinfectious cough,Su Huang cough capsule combined with montelukast sodium can effectively improve the immune function and lung function,reduce TCM symptoms such as cough,expectoration,reduce FeNO level and adverse incidence.
