摘要
目的 探讨盆底磁刺激联合手法治疗盆底肌筋膜疼痛综合征患者的疗效。方法 选取郑州大学第三附属医院2021年01~12月期间盆底康复治疗中心就诊的123例MPPS患者。将研究对象通过随机数表法分为磁刺激组(n=40例)、手法组(n=38例)和联合组(n=45例)3组。观察三组患者治疗前后的疼痛视觉模拟评分(Visual analogue scale,VAS)、Glazer评估指标的变化,评估治疗后的有效率及治疗结束1个月的疼痛复发情况。结果 治疗后,联合组的盆底肌筋膜VAS疼痛评分、静息电位肌电值均较治疗前、手法组、磁刺激组均改善明显(P<0.05),且手法组优于磁刺激组(P<0.05);联合组快速收缩阶段最大峰值、紧张收缩阶段及耐力收缩阶段平均波幅均较治疗前、手法组、磁刺激组上升显著(P<0.05),手法组与磁刺激相比,差异无统计学差异(P>0.05)。治疗后,联合组、手法组和磁刺激组的总有效率分别为97.78%,84.21%和72.50%,联合组的总有效率高于磁刺激组和手法组(P<0.05),磁刺激组和手法组比较差异无统计学意义(P>0.05)。治疗结束1个月后,联合组、手法组和磁刺激组的疼痛复发率分别为8.89%,18.42%和32.5%,联合组和手法组的复发率均低于磁刺激组(P<0.05),联合组和手法组比较,差异无统计学意义(P>0.05)。结论 盆底磁刺激联合手法是盆底肌筋膜疼痛综合征的一种有效的治疗方式,可以显著地改善患者的疼痛症状,纠正盆底肌过度活跃的功能状态。
Objective To explore the eficacy of pelvic floor magnetic stimulation combined with manipulation in the treatment of patients with myofascial pelvic pain syndrome(MPPS).Methods A total of 123 patients with MPPS treated in the Third afiliated Hospital of Zhengzhou University from January 2021 to December 2021 are selected and followed up successfully.The patients are randomly divided into a magnetic stimulation group(n=40),a manipulation group(n=38)and a combined group(n=45).The response to treatment is evaluated using a visual scale(VAS)to rate discomfort,plus a physical examination,and surface electromyography results using Glazer's protocol.The treatments'efficacy and the recurrence of pain are evaluated one month after the treatment.Result After treatment,the VAS score and EMG value of resting potential in the combined group are significantly improved as compared with those before treatment,the manipulation group and the magnetic stimulation group,and the manipulation group is superior to the magnetic stimulation group(P<0.05).The maximum peak of rapid contraction,the average amplitude of tension contraction and endurance contraction in the combination group are significantly higher than those before treatment,the manipulation group and the magnetic stimulation group(P<0.05),but there is no significant difference between the manipulation group and the magnetic stimulation group(P>0.05).After treatment,the total effective rates of the combined group,the manipulation group and the magnetic stimulation group are 97.78%,84.21%and 72.50%,respectively.The total effective rate of the combined group is higher than that of the magnetic stimulation group and the manipulation group(P<0.05).There is no significant difference between the magnetic stimulation group and the manipulation group(P>0.05).One month after treatment,the pain recurrence rates in the combination group,the manipulation group and the magnetic stimulation group are 8.89%,18.42%and 32.5%,respectively.The recurrence rates in the combination group and manipulation group are lower than those in the magnetic stimulation group(P<0.05).There is no significant diference between the combination group and the manipulation group(P>0.05).Conclusion Pelvic foor magnetic stimulation combined with manipulation is an effective treatment for pelvic floor myofascial pain syndrome,which can effectively improve the pain symptoms of patients and correct the functional state of pelvic floor muscle hyperactivity.
