摘要
目的:观察强肾汤对2—3期慢性肾脏病脾肾气虚证患者血清代谢组学的影响。方法:记录治疗前、治疗1周后35例2—3期慢性肾脏病脾肾气虚证患者临床资料,并对患者血清进行液相色谱-质谱联用检测,综合使用多变量统计分析方法[主成分分析(principal component analysis,PCA)、偏最小二乘判别分析(partial least squares discriminant analysis,PLS-DA)]探讨相关代谢标志物的水平变化及代谢通路的改变。结果:(1)35例患者经强肾汤药物治疗1周后的有效率为88.6%。(2)治疗1周后,患者尿蛋白定量、肾功能系列指标、血红蛋白、血浆白蛋白均显著改善,与治疗前比较,差异均有统计学意义(P<0.05)。(3)PCA、PLS-DA结果显示,患者治疗前后血清代谢表型比较,差异有统计学意义(P<0.05);鞘氨醇、甘油磷脂等12种生物标志物为强肾汤干预治疗后的差异性血清代谢物,所涉及的主要异常代谢通路可能有甘油磷脂代谢和鞘脂代谢。结论:强肾汤治疗2—3期慢性肾脏病脾肾气虚证的作用机制可能与改善鞘脂代谢、调节甘油磷脂代谢有关。
Objective:To observe the effect of Qiangshen Decoction on serum metabonomics in patients with spleen-kidney qi deficiency syndrome in stage 2-3 chronic kidney disease.Methods:The clinical data of 35 patients with spleen-kidney qi deficiency syndrome of stage 2-3 chronic kidney disease were recorded before and one week after treatment,and the serum of the patients was detected by liquid chromatography-mass spectrometry(LC-MS).Multivariate statistical analysis methods such as principal component analysis(PCA)and partial least square discriminant analysis(PLS-DA)were used to explore the changes of related metabolic markers and metabolic pathway.Results:(1)The effective rate of 135 patients treated with Qiangshen Decoction for one week was 88.6%.(2)After one week of treatment,the quantity of urinary protein,series indexes of renal function,hemoglobin and plasma albumin were significantly improved,which were significantly different from those before treatment(P<0.05).(3)The results of 3PCA and PLS-DA showed that there was no significant difference in serum metabolic phenotype before and after treatment,and 12 biomarkers related to Qiangshen Decoction were identified in serum(P>0.05).Conclusion:The mechanism of Qiangshen Decoction in the treatment of Spleen-Kidney Qi deficiency syndrome in stage 2-3 chronic kidney disease may be related to the improvement of sphingomyelin metabolism and the regulation of glycerolipid metabolism.
